Licensed Clinical Social Worker/LPC Job at Memphis Health Center
The Licensed Clinical Social Worker (LCSW)/LPC provides direct assessment, diagnosis formulation, treatment planning, treatment intervention and education to members and/or family members for behavioral healthcare.
ESSENTIAL DUTIES/RESPONSIBILITIES:
Provides timely and appropriate assessment, diagnosis, treatment planning & treatment intervention of members.
Consults with BHD and/or Psychiatrist on individual patients who do not improve and participate in clinical decision-making for stepped-up care and referral.
Conducts initial interview with Members by eliciting & interpreting the psychological & social meaning of complaints.
Maintains and builds linkages with County Mental Health, Alcohol & Other Drugs, Social Services, Hospitals and other community resources.
Prepares reports, intakes, treatment plans, and case summaries and maintains ongoing confidential records for the members.
Provides clinical consultation in an integrated health care environment regarding family interactions, socio-cultural determinants & community resources.
Assists the BHD in preparing reports on the behavioral health program for the CEO, community, donors and Board of Governors.
Develops and conducts health education/promotional presentations on health-related topics in community settings.
Plans and implements special community events and other programs including arranging for speakers, facilities, resources, and staffing.
Provides appropriate selection of treatment modality to member's presenting problem.
Demonstrates expertise in a variety of CBT-based treatment methodologies.
Determines urgency of member's need.
Provides treatment to children, adolescents, adults, and the elderly.
All other duties assigned.
EDUCATION/EXPERIENCE:
Master's degree in Social Work required from an accredited college or university.
Clinical Social Worker license in the State of Tennessee.
Must have a National Provider Identifier (NPI), or obtain an NPI, prior to employment start date.
Previous clinical responsibility to include crisis intervention, individual and group psychotherapy. Excellent interpersonal and communication skills. Demonstrated ability to effectively and sensitively provide care to people from different cultural groups.
Bilingual in Spanish and English, preferred.
