Job Title/Position: Clinical Manager
JOB DESCRIPTION SUMMARY
Responsible for assisting in the management of the Hospice team. A person in this position works closely with hospice staff, other managers, administrative staff, and patients.
Responsible for helping to maintain referral sources, responding to patient requests and concerns, managing patient care provided by the Hospice team, and other tasks.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Monitors evaluations, treatment protocol design, and treatment delivery for patients. Participates in the development of the total plan of care for patients.
- Continues to maintain an active patient caseload in conjunction with management responsibilities as needed.
- Ensures the hospice team follows regulatory compliance requirements and standard procedures.
- Establishes/reviews hospice policies, procedures, new programs, and related services; responds to requests and identifies emerging issues. Contributes information, analysis, and recommendations to strategic thinking and direction.
- Communicates on an ongoing basis and provides clinical and operational performance updates. Prepares and submits clinical and progress summaries based on the attainment of goals.
- Works with the Community Liaison team to increase patient referrals.
POSITION QUALIFICATIONS
- Licensed RN in the state of Ohio.
- 3-5 years of appropriate experience as a clinician. Community/home health experience preferred.
- 1-3 years of supervisory experience preferred.
- Possesses a diverse knowledge base and skill set to facilitate patient treatment, including considerable knowledge of the principles, methods, materials, and equipment of therapy.
- Ability to establish and maintain effective working relationships with professional employees and patients.
