SCOPE OF POSITION
The position description is only to be used as a guide. It does not constitute any type of contract between the agency and employee. At the agency’s discretion, the description may be changed and additional duties; qualifications or requirements may be added at any time. This position will demonstrate competency in the care of the (circle all that applies): Adolescent, Adult, and Geriatric patient population.
The Home Health Aide will be assigned to patients by a Registered Nurse. The Home Health Aide shall follow instructions provided by a Registered Nurse, Therapist or Nurse Supervisor as documented on the home health aide assignment sheet. Duties may include the performance of simple procedures as an extension of nursing or therapy services; personal care, ambulation and exercise, household services essential to health care at home, reporting changes in the patient’s conditions and needs, and completing appropriate records.
This position may be required to cross train within the agency. Responsibilities in secondary position may require access to medical records and other computer access exceeding routine computer usage. Access to patient information may be necessary to fulfill the requirements of this role; therefore, adherence to Ballad privacy and security policies and practices is a must.

EDUCATION EXPERIENCE
Demonstrated ability to read with comprehension, communicate in writing and calculate simple mathematical problems. Must have completed an approved Home Health Aide program of 75 hours, including 16 hours supervised practical experience as required by OBRA-87 laws or have completed Fundamentals of Nursing/one semester of an approved nursing school. Must successfully complete Agency Competency Evaluation. Prior hospital, nursing home or health aide experience preferred. Must have dependable transportation. Must carry current personal collision and liability insurance on any vehicle driven.
Must maintain current driver’s license in the appropriate state. Must possess and maintain current certification and cardiopulmonary resuscitation certification.
