We are currently hiring for a Care Manager in our Rochester Area! As a Care Manager at Venture Forthe, you will be responsible for the coordination of all medical and psychosocial care for the Health Homes consumer.

Job Qualifications

Two (2) years experience providing care management related activities. Internship experience can count towards the years of experience required.

Bachelor’s Degree in Social Work, Psychology, Sociology, Counseling, Rehabilitation or other Human Services related field preferred.

Responsibilities

Maintain a maximum caseload of 50 clients with a minimum of one monthly contact with each. This can include family members and service providers
Assess to identify problem areas to assist in case management care plan
Assess immediate needs, develop and implement action plan if necessary
Provide comprehensive care coordination and treatment planning
Participate in transitional care from inpatient settings to community, including post discharge follow up
Coordinate with service providers and informal supports involved in care
Knowledge of community resources including Social Services and SSA
Maintain documentation of outreach process for each client, including but not limited to progress notes, assessments and contacts with all service providers
May have 24/7 on call responsibilities

Specialized Skills

Ability to manage time management efficiently
Excellent written and verbal communication skills
Demonstrate ability to interact with community providers and colleagues professionally
Proficiency with Microsoft Office and G-MAIL
Ability to meet and maintain deadlines
