Overview:
Provide patients and their caregivers treatment and education on their conditions to help ensure a proper recovery.
Provide admission, case management, and follow-up skilled nursing visits for home health patients.Provide on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Supervisor. Conference with physician to develop the initial plan of care and have continual dialogue to update the plan of care as necessary. Coordinate with other disciplines of care such as Physical Therapists, Occupational Therapists, MSW, Home Health Aides and external providers. Participating in the Quality Assurance/Quality Improvement Process. Adhering to all practice standards as they apply to patient care. RN Case Managers will be expected to perform the following:
Provide admission, case management, and follow-up skilled nursing visits for home health patients.Provide on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Supervisor. Conference with physician to develop the initial plan of care and have continual dialogue to update the plan of care as necessary. Coordinate with other disciplines of care such as Physical Therapists, Occupational Therapists, MSW, Home Health Aides and external providers. Participating in the Quality Assurance/Quality Improvement Process. Adhering to all practice standards as they apply to patient care. Such other tasks as necessary to provide appropriate care for the patients.
- Furnishes those services requiring substantial and specialized nursing skill that are ordered by the physician as indicated in the plan of care.
- Counsels the patient, caregiver and family in meeting nursing and related needs.
- Uses infection control measures that protect both the staff and the patient.
- Coordinates services. Informs the physician and other personnel of changes in the patient’s condition and needs.
- Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
- Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician.

Qualifications:
- Minimum Associate Nursing degree.
- Valid Mississippi RN license.
- Valid driver's license.
- Ability to successfully complete required background check and drug screen.
Required Experience
- Minimum 1 year acute care nursing experience, home health experience preferred.
