The HIV/AIDS Program (HAP) Coordinator manages administrative responsibilities, e.g. data collection and analysis, report preparation, budgeting, grant writing, and staff coordination.

The HAP Coordinator performs and/or supervises the following activities.

1. Review and assign new referrals to medical case managers. Document the new referrals and assignments as required by the funder(s).

2. Provide Information and Referral services to assist with agency Intake and Admission procedures and provide and/or supervise Medical Case Management services to clients, including but not limited to, comprehensive bio-psychosocial needs assessment, service planning, service linkages and monitoring, linkages with medical providers, discharge and aftercare/follow-up planning, and advocacy. Depending upon contractual and/or program requirements and prior training, the HAP Coordinator may be required to provide, and/or supervise, specialized assessments.

3. Provide daily oversight and consultation to HAP staff regarding progress or problems with assigned cases and administrative tasks.

4. Instruct HAP staff in agency policy and procedures and assist staff confronted with dilemmas to interpret applications of policy or to obtain assistance from agency management. Delegate appropriate activities to HAP staff and coordinates/monitor such activities. Keep agency management informed through the immediate supervisor and through administrative meetings regarding staff activities, performance, and any problems with operations.

5. Prepare/present oral/written program reports as required as contract liaison, which include narrative/data descriptions of services, progress, and problems with recommendations for program changes or improvements.

6. Represent the agency to the community and other organizations through activities of public speaking, education, networking, etc.

7. Facilitate timely response by the agency and project staff to special service requests made by the community, other organizations, etc.

8. Take responsibility to promote and develop professional relationships and problem-solving among staff and communicate proactively with other service providers, especially at community-based service sites. Demonstrate tact and resourcefulness in promoting interagency collaboration.

9. Complete and maintain orderly, legible, timely records of services and other documents in narrative or data formats as required by the Agency or its contracts. Prepare high quality written drafts of program reports for third parties.

10. Understands and completes periodic performance evaluations and other documentation as required for personnel management.

11. Understand and observe each client's rights to privacy and confidentiality as articulated by agency policy and procedures -- particularly for special programs, projects, services regarding HIV/AIDS privacy and confidentiality.

12. Manage grants and funding requirements, including reports, program requirements, grant proposal writing for re-funding or new program funding, budget management, and careful attention to monthly goals for each grant.

QUALIFICATIONS: Master of Social Work or Master Degree in a closely related health or human service profession from an accredited college or university. Five or more years of direct service practice and supervisory experience. Knowledge of HIV/AIDS treatment and prevention is desired.

Candidate must be able to pass the following:

  • FBI Fingerprinting Clearance
  • Pennsylvania Criminal Clearance
  • Pennsylvania Child Abuse Clearance
  • Drug-Screen
