POSITION TITLE: Health Navigator
PROGRAM: Supportive Services for Veteran Families (SSVF)
DIVISION: Veteran Services
REPORTS TO: Program Manager
SCOPE: The SSVF Health Navigator works closely with the Veteran’s SSVF Case Manager, multidisciplinary team, and medical specialists to create an enhanced care coordination plan for the Veteran’s specific needs. The Navigator will act as a liaison between SSVF team and the VA and/or community medical clinics.
Job duties include:
Provide care coordination; assisting in scheduling Primary Care, Specialty Care, Mental Health, and Substance Use Disorder appointments and ensuring the Veteran has appropriate transportation to and from these appointments.
Use strengths-based approach; work closely with the Veteran to assist them in communicating their preferences in care and their personal health-care related goals. Serve as a resource for education and support for Veterans and families in identifying goals, resources, and tools to meet those goals.
Maintain accurate and up to date, comprehensive case notes with strict adherence to HIPPA standards.
Use Health Literacy principles; the Healthcare Navigator explains clinical information and provides recommendations to Veterans and families using clear language, at the appropriate literacy level, and assists the Veteran in identifying questions and concerns for their provider.
Provide system navigation both within the VA healthcare system and Community healthcare; scheduling and attending healthcare and community resource appointments with the Veteran and providing post-appointment education and goal setting to ensure the Veteran is in alignment with and following the agreed upon treatment plan.
Connect Veterans to VA healthcare benefits or community healthcare services (Medicaid/Medicare) when Veterans are not eligible for VA healthcare.
Receive training to become a Certified SSI/SSDI Outreach, Access and Recovery (SOAR) Specialist. Work with Veterans and their families to apply for SSI/SSDI benefits.
Act as an advocate for the Veteran to ensure that Veteran goals and preferences are included in care plans and communicate with healthcare providers and treatment teams to ensure an understanding of treatment plans and work collaboratively with the Veteran and treatment team in adherence to this plan.
Maintain courteous and cooperative relationships with supervisors, co-workers, Veterans, visitors, and the public contributing to the effective operation of the case management program. Anticipate and avoid potential causes of conflict, and actively promote cooperation among co-workers.
Effectively communicate with and utilize community agencies to facilitate continuity of care. Maintain regular contact and interaction with a variety of community agencies and resources. Collaborate with a variety of community service agencies and engage in problem resolution activities.
Develop and maintain awareness/marketing strategies to inform the public of SSVF services available and to engage each community partner in support of the program while building a referral network. Assist with resource development and events as requested.
Stay informed of agency and program information through inter-office communications, attendance at staff meetings, participation in professional development activities and other Easterseals Oregon events.
Complete other assignments as requested by the Director of Veteran’s Services and Program Manager.Minimum Education, Certification, Experience:
* Bachelors’ degree in social work, human services, related field, or 2 years' experience working with Veterans.
* Demonstrated interest in working with Veterans and the ability to be compassionate and understanding.
* Demonstrated ability to work with others and communicate effectively.
* Ability to work with individuals of varying socioeconomic status, skill levels, experiences and cultures.
* Availability to work a non-traditional schedule, if needed (possible holidays, evenings and weekends on occasion).
* Maintenance of a professional appearance.
* Strong Information technology skills.
* Ability to manage multiple tasks simultaneously.

Pay: From $20.00 per hour
