Geriatric Care Manager Job at Sage Solutions, LLC
Sage Solutions, LLC is a 20-year-old successful geriatric care management and consulting practice headquartered in West Hartford, CT. We are seeking an experienced nurse, social worker, or care manager to help support our existing team. This is part-time, per diem contract work with the possibility of future employment. Hours fluctuate widely and can largely be set by the care manager.
Candidates must have at least 10 years experience in geriatric care management, social work, or nursing, possess a minimum of a bachelor's degree and applicable certifications/licenses, and be able to assess the needs of older adults and their families as they navigate the challenges of aging. Direct experience with older adults is REQUIRED; experience with individuals with memory impairment is a plus. Patience, compassion, organization, collaboration, and timeliness are all keys to performing this work. Hourly rate is commensurate with experience.
