Family Support Worker Job at Southern New Jersey Perinatal Cooperative

The Family Support Worker (FSW) is responsible for initiating and maintaining regular, long-term contact with families. During these visits, FSWs concentrate on family-centered, strength-based, goal-oriented services in order to assist families with issues involving the parent-child relationship, improving the family support system, family problem solving, and advocating for the client. Referral and monitoring of health care usage and child development are also stressed.
Essential Functions:
- Provides a range of culturally competent home-based services on a prescribed basis for families participating in the program including: parenting education, developmental milestones, health and nutrition education, home management skills, crisis intervention and social services linkage, all in partnership with the client
- Identifies and refers clients to other support services in the community, assists in making and attending health and human service appointments, including appointments related to educational and employment goals
- Completes initial assessments and submits all documentation within program guideline timelines
- Meets productivity requirements as required by program
- Guides families in the development of Individual Family Support Plans (IFSP) based on strengths and needs of the individual families, thereby helping families establish measurable and attainable goals
- Develops and maintains ongoing outreach strategies to inform families and the community of Healthy Families and Infant and Family Development programs
- Ensures confidentiality of client records
- Meets with Supervisor per program requirements to report and discuss work
- Attends orientation and ongoing training sessions on maternal and child health, welfare, housing nutrition, substance abuse, counseling, parenting, infant and family support issues
- Performs other duties as assigned
Knowledge, Skills, and Abilities to Perform Essential Functions:
- Associate’s degree preferred or participation in courses towards an Associate’s degree or completion of a Community Worker Certification course
- Valid driver’s license, automobile with current insurance
- Knowledgeable about community resources
- Ability to work well with a variety of people and develop rapport with clients from varied cultural/ethnic backgrounds
- Effective oral communication skills; ability to keep written records
- Bilingual in English/Spanish (preferred)
Physical Requirements:
- Must be able to see to read documents and hear well enough to communicate with others
- Must be able to write, type on a computer, and use a telephone
- Must be able to operate a motor vehicle
- Must be able to lift up to 20 pounds
- Additional requirements to be determined with input from Director and HR

Work Environment:
Travels regularly to sites where traffic, weather, and other environmental conditions may be encountered; otherwise works in a climate-controlled professional office environment generally free from hazards and other adverse working conditions
