Performs casework duties under general supervision; does related work as required.

Conduct home visits with health department staff to complete long-term care screenings by interviewing adults to determine eligibility for services, and complete all necessary documentation for case approvals and appeals; review initial screening requests to determine appropriate screening process; coordinate with the human services program staff and other partners to assure that their processes are complete and customers can be served as determined by the screening process; participate in team meetings to address the screening process.

Assist customers with the Medicaid application process by helping them fill out applications, sending out verification checklists, and collecting verifications for the Medicaid determination. Follow-up after a screening is approved for services to assure they are put in place. Regular coordination with human services professionals internally and across agencies to complete the screening process and assist with services implementation. Be knowledgeable about agency and community programs to provide referrals to both internal and external customers.

Ability to search automated systems for customer information, and enter screenings information into applicable state database timely. Complete all case documentation and case records in accordance with State and local policies and procedures, including all appropriate and required state and local forms; submit forms to meet deadlines; verify and document all service provision data; initiate needed correspondence with all appropriate parties in a timely manner. Complete and maintain accurate case records; keep filing current and records organized.

Participate in community, agency and division committees and task forces as needed for specific projects, or for ongoing functions.

Mandated participation on a Henrico County Shelter Team with assignments as designated by the Emergency Operations Center and county leadership. Perform related work as assigned. Accept additional assignments when necessary.

Considerable knowledge of social work principles and practices; considerable knowledge of current social, economic and health concerns and problems, both globally and locally; good knowledge of individual and group behavior and of current literature and trends in the social work field; good knowledge of community resources available to meet client needs; ability to make independent judgments and to work with a minimum of supervision; ability to gather and successfully assess information for application real-Ire situations; ability to make and implement case decisions; ability to work with clients with tolerance and objectivity; ability to understand and interpret regulations and laws; skill in planning and organizing work; ability to secure the cooperation of others; ability to communicate effectively, orally and in writing.
