Discharge Planner Job at Erlanger Behavioral Health Hospital
The Discharge Planner provides assistance to patients and their families by developing a documented continuing care plan to ensure ongoing support in recovery and or other behavioral, mental or health care follow up after discharge from the facility. Develops continuing after care plans, based on patient needs. Responsible for setting discharge appointments for patients, including group or individual therapy. Providing all needed documentation to other providers. Acts as a liaison between referral sources, patients and counselors. Ensures all patient discharge planning documentation is clear, concise and timely.

Pay: $37,000.00 - $40,000.00 per year
