MAJOR AREAS OF RESPONSIBILTY:
- Plans, designs, and implements day-to-day participant services in assigned Division/worksite.
- Develops the overall master schedule for program activities and specific job contracts, staff assignments, and work schedules and coordinates with other Designated Coordinators.
- Provides direct coordination of a caseload up to 40 participants that includes the following:
- Possesses a thorough understanding and implements effectively the designated compliance aspects of MN SS245D and associated Statutes and Rules, PHASE Policies and Procedures, and all other applicable Federal, State and local regulations related to service provision.
- Acting as an effective member of the EST to ensure the implementation of, and training on, participant-first and participant-directed program plans.
- Coordinates annual goals and objectives with the “Q” and other EST members for all participants on assigned caseload.
- Provides oversight of the specified/assigned responsibilities outlined in the CSSP and CSSP Addendum.
- Provides instruction to participants individually or in groups.
- Ensures all required and needed data is recorded daily.
- Assists the “Q” in the development/collection/reporting of participant specific data for all participant assessments and plans. Including but not limited to IAPPs, CSSP Addendums, SMAs and Outcome plans.
- Ensures all direct support staff working with assigned participants are trained according to the participant’s CSSP Addendum, and PHASE policy/procedures.
- Assists the “Q” in the development of person-centered plans that include support strategies that assess the participant’s strengths, needs, and preferences.
- Assists the “Q” as needed in the development of Functional Behavioral Assessments.
- Ensures the administration of participant medication and maintains appropriate records consistent with PHASE policies and procedures.
- Provides direct hands-on training to DSPs in accordance with PHASE Training Policy and required forms.
- Maintains on-going communications with other service providers (i.e., County Human Services, Residential Providers, community job sites, primary caregivers, etc.) to ensure consistency of services and to be informed of changes that may affect the participant’s continuity of care.
- Provides direct supervision to direct support paraprofessionals. Recommends and assists in selection of staff, orients and assigns job duties, coordinates staff training, required meetings, evaluates performance, develops goals to improve performance and provides personnel problem solving.
- Coordinates with other staff development of training programs that are consistent with participant’s plan.
- Provides training to DSPs and the observation of the specific staff interventions identified in Positive Support Transition Plans.
- Operates PHASE vehicles in transporting persons to and from various community destinations consistent with PHASE transportation and safety policies. May be required to use personal automobile for agency transportation needs.
- Attends, coordinates and participates in staff meetings, community meetings/events, and in-service trainings, workshops, and classes.
- Assists with the vocational/program assessment for all new participants in employment/program service settings including developing on-site procedures for assessing program participant’s job skills.
- Assists in developing plans and strategies to address vulnerability of person served.
- Conducts job/task analysis to determine job-training requirements. Suggests strategies for job accommodation or modification to better fit person’s skills, abilities, and interests.
- Performs time studies for wage determination to meet Department of Labor requirements.
- Provides direct on-site training and instruction to program participants to develop required skills to perform assigned job duties
- Monitors program participant job performance and provides positive support to the participants.
- Monitors participant’s skill development to ensure satisfactory job performance. Assists in establishing criteria for satisfactory performance.
- Maintains work logs and individual hours and timecards.
- Assigns DSPs and participants specific job duties within time allocated and consistent with PHASE schedules and operations.
- Works with job site organizations/employers to ensure work performance is adequate for their needs and assures job completion to their satisfaction.
- Responsible for maintaining standards of cleanliness, safety, décor, stocking of first aid kits, planned organization and effective workflow in assigned physical programming / work spaces. Makes immediate recommendations to direct supervisor for adaptations when assessed
- Attains certification and provides First-Aid and emergency CPR to persons as needed.
- Complies with PHASE policies and procedures.
- Keeps Director/” Q” informed of important developments, potential problems, and related information necessary for effective program management. Coordinates program plans and activities with other staff as appropriate, to ensure a coordinated work effort and team approach.
- Performs related work as apparent or assigned.
- Develop and maintain systems that provide payroll documentation to the Finance Department for participants and staff members.
- Conducts department meetings and formulates agendas. Develops necessary ongoing training for staff as it relates to changes that occur in the individual participant program plans.
- Act as a member of various committees and chair committees as assigned.
- May provide tours of the building and operations of PHASE to community members, stakeholders and licensors.
- Coordinates services for community contract work and acts as the contract representative for PHASE.
- Completes required training and ensures the documentation of training is retained in the training records file.
- Provides direct care, on-site training and instruction to participants to develop required skills to perform assigned job duties.
- May perform the duties of a DSP position, as outlined in the respective job description.
QUALIFICATIONS FOR ENTRY:
- AA degree equivalent in special education, human services, or related field.
- Designated Coordinator status qualified.
- At least 2 years of professional experience in human service agency environment with persons with disabilities.
- Two years’ experience managing employees.
- Philosophy consistent with the mission and purpose of PHASE.
- Good driving record and the possession of a current Minnesota driver’s license.
- Ability to pass criminal background checks requirements, vehicle insurance qualifications, and medical examination requirements (with doctor certification).
