General Description
The Direct Care Coordinator (DCC) is directly responsible for oversight of a caseload within the Home-Based Therapeutic Services (HBTS) program, Personal Assistance Service and Supports (PASS) program and/or the Applied Behavior Analysis (ABA) program. This includes proactive planning, quality assurance, and outcome-oriented services. Some core tasks include staff supervision, training, and program monitoring as outlined in each program’s State of Rhode Island Agency Certification Regulations. This also includes completing Direct Support Professional Hours within the home.
The Direct Care Coordinator reports directly to and receives weekly and/or as needed support and supervision from the Program Coordinator(s) and Clinical Supervisor(s) of Children’s Program. The Children’s Direct Care Coordinator position works in accordance with the policies and procedures of The Arc of Bristol County, Inc. d.b.a. proAbility, The State of Rhode Island’s Department of Human Services, and all other funding sources to oversee and coordinate individualized services to children with special healthcare needs and their families.
Job Responsibilities
Provide in-home support to families on caseload (Provide in Home Direct Support Professional Hours for families on caseload.)
Implement interventions as directed by the Clinical Supervisor(s) and treatment plan
Utilize de-escalation strategies with client as necessary
Document data from in-home supports and turn in to supervisor by required deadline
Complete required agency trainings in a timely manner
Track and calculate client data for preparation of the treatment plan
Supervise assigned direct support staff including training, scheduling, discipline, and performance reviews. Maintain accuracy and timeliness in the review of time taken (sick, vacation, etc.) and submission of supervised timesheets on a weekly/bi-weekly basis.
Recruit and train staff in accordance with agency and state policies and procedures. In collaboration with the Program Coordinator(s) and Clinical Supervisor(s), provide staff training, mentoring, performance evaluation and disciplinary action as appropriate to specific services (HBTS/PASS/ABA).
Meet with staff regarding individual plan(s) of service. Conduct monthly (minimum) home visits with each family on caseload.
Oversee case files and review case plans and progress reports for quality assurance and to adhere to state regulations regarding Record Management. Oversee submission deadlines and ensure timely submission of all new plans, renewal plans, and progress notes.
Comply with State regulations regarding referrals and intakes, implementing state and agency intake policies to respond to a request for services.
Provide quality assurance/oversight to the delivery of coordination services for individuals in HBTS, ABA and PASS programs.
Uphold and comply with all agency policies and safety programs to maintain a safe work environment for employees, individuals, family members, and visitors. Adhere to all other agency guidelines and department specific policies
Protect the rights and dignities of individuals with developmental and other disabilities and extend these rights and dignities to family members or guardians.
Represent The Arc of Bristol County/proAbility in the community by representing the agency in a positive and supportive manner within the community and with the individuals and families we support.
Commit to organizational quality initiatives by participating in programs that will enhance treatment services, improve team processes, which includes but is not limited to regular attendance at staff meetings.
Perform other duties as assigned.
Minimum Qualifications
- Bachelor’s degree in related field of study
- 3 years of experience working with individuals with developmental/intellectual disabilities (preferred)
- 1 year of experience working as a HBTS/PASS/ABA Treatment Coordinator (preferred)
and/or
- 1 year of experience working as an HBTS/PASS/ABA Direct Support Professional
- Experience in advocacy for individuals with disabilities
- Safe driving record, current driver’s license, and motor vehicle insurance
Job Type: Part-time
