Function:

Under the direct supervision of the Adult Day Supports Director, identifies assessment based training objectives that help promote independence of the individuals receiving services. Develops service plans, training interventions, maintains records, and provides transportation for the individuals participating in the Day Service.

Job Duties:

1. Monitors the progress of people receiving services, completes progress notes for individuals attending the Day Program to Service Coordination by the end of each month.

A. Monitors training intervention data monthly, determines progress or lack of progress.

B. Documents progress assessment in notes by the 10th day of the month for the previous month.

C. Provides monthly progress notes to Service Coordinators.

D. When progress has not been made for 2 consecutive months, provides explanation and revises the training intervention to improve the likelihood of progress occurring.

E. Makes a copy of monitoring notes available for service coordinators.

2. Completes or ensures that assessments are completed within 30 days of entry into the service and annually thereafter prior to a day service planning meeting.

A. Provides staff with the necessary tools to complete an assessment, including forms, training and consultation regarding needed corrections.

B. Assessments are distributed to assigned employees no less than 30 days prior to the plan meeting date.

C. Ensures the assessment is returned completed no less than 5 work days prior to the plan meeting date, notifying the supervisor of late assessments.

D. Assessments address the required areas of skills identified in the Day Services Guidelines.

3.Responsible for the review of and filing of Monthly Data Recording Sheets, and assessments, Plans, and other documents, for legibility, completeness, signature and date, and informing supervisors of incomplete or missing documents.

A. Reviews data records as they are submitted to assure data is available for each unit of service reported.

B. Reviews assessments and other documents as submitted to assure completeness and that written information is legible, with signature and date, before filing..

C. Ensures that staff has written comments documenting absences, late arrivals and early departures, as well as explanations for data discrepancies, on both the data and assessment forms before filing.

4. Maintains each individual file in a neat and orderly fashion, according to the approved file format. Must file all documents in a timely manner.

A. Responsible for maintaining a file for each individual enrolled in the Day Program.

B. Ensures that each file contains the appropriate and required documentation.

C. Ensures filing is consistent in folders and is completed according to the file format approved by supervisor.

D. Ensures that each file has current information and at least one full year of information is maintained in each person’s chart.

E. Ensures that appropriate documents are stamped “Do Not Remove” and ensures that documents are maintained in the files at all times.

F. Ensure that records are kept in chronological order, with the most recent information first in each section according to the file format.

5. Coordinates and leads intake meetings, providing explanation of services, procedures, and forms presented, and ensures that all initial documentation required for entry is signed and dated.

A. Ensures that all documentation, including two step TB, Medicaid Card, directions to Home, birth certificate, etc., is attained at the intake meeting.

B. Ensures that appropriate signatures are received from the parents/guardians or individual.

C. Through discussions with service coordinators and families, assures authorizations for services are received and establishes an entry date for the individuals into the Day Program.

D. Develops and has available to direct support staff the preliminary Plan of Service, to include health information necessary for appropriate care, supervision plans for the settings in which Day services will be provided, and the plan for assessment of skills.

6. Develops and monitors the effectiveness of training interventions to meet the goals of the persons served.

A. Within 3 days of the plan meeting, prepares Monthly Data Recording sheets for the goals and objectives identified at the plan meeting.

B. By the end of the 3rd day from the plan meeting, Monthly Data Recording sheets and training procedures and materials needed for training implementation are provided to the responsible direct support staff.

C. Provides training materials and activities to each training area (class) to enhance acquisition of targeted functional skills for the service area.

D. When no progress is being made as determined by the monthly review of progress, within 2 days of the determination of no progress, develops and makes available to direct support staff revisions to the training procedure or to the objective if indicated.

7. Conducts plan meetings with those being served and prepares the Plan of Services for Day Supports.

A. Within 30 days of entry and annually thereafter, develops the Plan of Service based on strengths and needs identified through the initial and annual service assessment.

B. The Plan of Service for each person served includes goals, objectives, supervision strategy, emergency information, health information, and any additional support information needed for the person.

C. With the consent of the person served, invites involved caregivers or family members, and invites guardians, to plan meetings or makes written or phone contact as needed regarding the plan developed.

D. Creates a tickler system to ensure that plans are completed in a timely manner.

E. Prepares reports for and participates in ICF/MR service participant plan meetings.

F. Amendments to Plans are completed as goals, objectives, services are changed or updated or when significant changes in the person’s Day Service participation occur.

8. Coordinates with Behavior Support providers, therapists, and residential services to assure data collection for other issues for the persons receiving services is accurately maintained.

A. Maintains contact with other service providers to gather current data collection forms and makes these available to responsible direct support staff.

B. Reviews documents submitted monthly, prior to forwarding to the appropriate provider, to assure that signatures, dates, and comments are included, makes copies filing, files, and informs Day Program Supervisor if issues are identified.

9. Serves as a van driver.

A. Transports consumers from their homes to the Day Program.

B. Transports consumers from the Day Program to their homes.

C. Transports consumers from their homes to work.

D. Transports consumers from work to their homes.

E. Transports consumers that participate on enclaves.

10. Other related duties and responsibilities.

A. Attends and participates in discussions for assigned planning committees.

B. Serves as a trainer in general orientation when assigned.

C. Serves as a backup resource for situations where staff are needed to implement planned services.

D. Performs other duties as assigned in a timely and accurate manner.

Minimum Requirements: Associates degree and three years of experience working with individuals with special needs; a valid driver’s license.

Physical Requirements: Reading, writing, driving, standing, walking, sitting, lifting, carrying, pushing, pulling, climbing, balancing, stooping, kneeling, crouching, crawling, reaching, handling, speaking, hearing, seeing, depth perception and color vision.

Skills and Knowledge Requirements: Must have strong writing, organization and communication skills. Must be able to remain focused on tasks. Must have knowledge of writing objectives and monitoring data. Must be able to read and interpret assessments. Must have strong computer skills.
