Job Summary and Qualifications

Our Social Workers evaluate the psychosocial needs, crisis intervention, development and implementation of appropriate, timely plans of care to include discharge planning for the assigned patient population. The commitment to service, safety and professionalism is critical to achieving the best experience for our patients. What You Will Do In This Role:

• You will meet with new patients to complete the psychosocial assessment.
• You will manage the utilization of services through concurrent reviews with payors and/or through internal reviews.
• You will coordinate discharge planning activities.
  

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Satori Alternatives Manager must be obtained within 30 days of employment start date
• Licensed Professional Counselor, or Licensed Masters Social Worker (LMSW), or License Professional Counselor Intern, or Registered Nurse, or License Clinical Social Worker (LCSW)

This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
