We are currently searching for a dynamic individual to become the Complex Case Manager for our Lead Program.

This position is responsible for:
  • Acting as the contact point for all referrals to LEAD case management
  • Managing referrals based on capacity
  • Ensuring effective and efficient communication and collaboration between all the partners involved in referring and receiving referrals for LEAD
  • Providing screening, outreach, and engagement to individuals referred to LEAD
  • Providing street outreach as needed to engage referred individuals who have yet to engage with their assigned case manager
  • Expanding outreach to LEAD-eligible individuals in Pueblo County
  • Developing and maintaining positive, collaborative relationships and communication throughout the community, law enforcement agencies,and community-based service providers in order to best serve LEAD participants
  • Facilitating communication and compliance with data-sharing between participating agencies and community-based service providers ensuring the fidelity of data
  • Facilitating training between participating agencies and community-based service agencies by subject matter experts
  • Assisting Project Manager with public information and community-awareness events


ESSENTIAL DUTIES AND RESPONSIBILITIES:

    • Serve as point of contact for LEAD referrals
    • Provide complex case management services to LEAD participants
    • Complete LEAD candidate intake and psycho-social assessment as needed
    • Manage referrals based on capacity
    • Manage and coordinate the assignment of new referrals to a case manager
    • Provide street outreach to engage individuals who have been referred to LEAD with the purpose of facilitating the completion of the intake process
    • Engage LEAD participants through outreach, trusting relationships, and individually tailored case management services
    • Collaborate with LEAD participants to develop an individualized service plan and help participants achieve identified goals
    • Advocate for LEAD clients to gain access to a wide variety of community resources
    • Connect LEAD clients with community resources
    • Identify gaps and barriers in available community resources and advocate for systemic changes
    • Serve as the point of contact and community connector between active LEAD participants and service agencies to assess the effectiveness of services provided to participants
    • Develop and maintain positive, collaborative relationships and communication throughout the community, law enforcement agencies, and community-based service providers
    • Track and enter all program referrals into the database (NEON). Update demographic information, referral date, type of referral, arrest charge, and case manager assignment
    • Develop and maintain patient files for assigned caseload according to agency and contract requirements. Enter all LEAD-specific data and information generated throughout the case management process, which may include follow-up with referral sources and documenting the outcomes of LEAD participants
    • Work in tandem with community-based referral services to provide street outreach to engage participants when needed
    • Develop cooperative relationships with Pueblo County Sheriff’s Office, Pueblo Police Department, and other outreach programs to coordinate services
    • Develop and maintain positive, collaborative relationships with LEAD partners and other service providers including chemical dependency treatment providers, mental health providers, health care providers, shelter providers, landlords, detox centers, and DSS workers. Attend all LEAD Program staff meetings and other required meetings
    • Participate in Operational Workgroup meetings.
    • Document all activities according to agency and contract requirements.
    • Participate in the Continuum of Care meetings to keep current on available community-based resources and how to access the services; report this information to the Operational Workgroup.
    • Work with IT providers and partner agencies to serve as multi-agency data coordinator, ensuring data is collected and entered in a uniform matter to ensure fidelity of data. Provide training and support to agencies for data collection.
    • Work with Project Manager to analyze and report data by specified deadline dates.
    • Identify and schedule ongoing training with subject matter experts for Harm Reduction and LEAD Principles for delivery to law enforcement, service providers, and Community.
    • Develop innovative interventions for LEAD participants.
    • Participate in public information and community-awareness events.
    • Additional duties as assigned.

SUPERVISORY RESPONSIBILITIES: N/A

QUALIFICATIONS:

Education
:
Associate’s degree required. Bachelor’s degree in a related field preferred.
Must be able to earn CAT within 18 months of hire.

Experience
:
At least two (2) years of experience in the behavioral health or human services field.

Other
:
  • Experience in data collection, case management, and clients struggling with SUD.
  • Ability to engage in strategic communications, and work with counselors, case managers, as well as the public.
  • Experience developing safety plans
  • Ability to work with diverse populations in various states of anxiety, sobriety, and aggression
  • Ability to verbally de-escalate individuals
  • Ability to visually assess for mental or physical health crisis
  • Ability to quickly problem-solve and make decisions
  • Computer literate with basic knowledge of Microsoft Office Suite
  • Must possess a valid driver's license and be insurable
PHYSICAL DEMANDS:

WORK ENVIRONMENT
:

Typically work standard office hours, but during busy periods may be required to work additional hours, during the evenings or even on the weekends. Responsible to attend: networking events, launches, and exhibitions which usually happen outside office hours.
