General Purpose
Under direction of the Community Referral Coordinator, the Community Referral Technician will work with Mountain Family Health Centers patients and staff to identify social determinants of health needs that prevent them from obtaining healthcare or barriers that are preventing the patient from achieving whole health. Supporting the MFHC Strategic Plan Goal 1 to improve the health of the population we serve; this position will educate them on community resources and connect them through the CRN platform.

Job Qualifications*

• Minimum education level of high school degree or equivalent, one year experience in customer service or equivalent in work experience and continuing education.
• Strong desire to help improve the health of your community.
• Basic knowledge of the E.M.R and clinical templates preferred
• Bilingual English to Spanish is preferred but not required.
• Excellent written and oral communication skills, including English usage, grammar, punctuation, and style.
• Ability to use designated reference materials.
• Ability to work independently with minimal supervision.
• Ability to prioritize and perform multiple tasks.
• Ability to work under pressure in a fast-paced environment with time constraints.
• Ability to understand and present verbal instructions and to exchange verbal information.
• Ability to work in Windows based computer environment.
• Ability to work in professional manner.
• Ability to travel when needed.
