Through trauma-informed and healing-centered engagement programming, CHC believes in the power of partnering with California's underserved communities to advance health equity and social justice across the state of California.

SUMMARY

With support from the Project Director and Program Manager, the Community Engagement Coordinator will take a lead role in the engagement and coordination of the Community Health Worker (CHW) network and community outreach activities in Kings County. The Kings County Healthy Communities Project aims to implement services and interventions supporting the prevention and spread of COVID-19, particularly among underserved populations most affected by COVID-19. This will be accomplished by coordinating and participating in community outreach activities and events, hosting COVID-19 Polymerase Chain Reaction (PCR) testing and vaccination events, and developing a Promotor/Community Health Worker Education Network. Additionally, this role will support in creating health and community linkages to the public based on social determinates of health (SDoH) screenings conducted by the program.

ESSENTIAL DUTIES

1. Consults with the Project Director and Program Manager relative to planning and program direction.

2. Responsible for creating safe and inclusive environments for Promotores/Community Health Workers.

3. Present and model values and behavior consistent with inclusivity, respect, and affirmation of all BIPOC identities.

4. Develop and implement a community outreach plan with the assistance of CHWs that outlines community outreach instances and community linkages for Kings County residents to connect them with resources and reduce health and SDoH disparities.

5. Coordinate monthly trainings and meetings with CHWs to determine successes, challenges and opportunities for growth.

6. Support the Program Manager with coordination between the subcontractors to identify roles and responsibilities to successfully complete program activities.

7. Maintain a log of activities and collect data necessary for program reporting and evaluation.

8. Support CHWs with screening and referring community members to services and resources based on their stated needs.

9. Develop community partnerships to support the goals of the program, including establishing relationships with program subcontractors, community members, and organizational partners.

10. With support from the Director and Program Manager, conduct ongoing evaluation and prepare required monthly and annual reports for the project.

11. Represents the California Health Collaborative at meetings with committees, community agencies, care providers and related entities, and facilitates inter-agency linkages, communication, and collaboration.

12. Performs other duties as assigned.

EDUCATION AND/OR EXPERIENCE

Education and experience requirements are a bachelor’s degree in Community Health, Child Development, Social Work or a related field and a minimum of two years’ experience working with diverse families; OR A combination of work experience and education, indicating an ability to fulfill the requirements of the position. One years related experience may be substituted for each year of college credit. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

A valid California Driver’s License and proof of insurance.

LANGUAGE SKILLS

Ability to read, analyze and interpret technical procedures, procedure manuals, government regulations, technical articles, and general business periodicals. Ability to write in an effective, concise manner. Ability to effectively present information to groups of various backgrounds and respond to questions.

REASONING ABILITY

Ability to work independently, exercise initiative and good judgment, solve practical problems and deal with a variety of variables in situations where only limited or no standardization exists, and interpret a variety of instructions furnished in written, oral, diagram or schedule form. Ability to create safe and inclusive spaces for all youth participants.

OTHER KNOWLEDGE/SKILL/ABILITIES

Ability to communicate effectively in both written and oral form.

Ability to travel and, if necessary, attend out of town meetings or conferences which could require overnight travel.
