CNA - Certified Nursing Assistant Job at Northland PACE
Job Summary: Provides physical assistance and attendant care to participants across care settings (center, home, transportation). Provides assistance with Instrumental Activities of Daily Living (IADLs). Provides assistance with nursing care, restorative therapies, activities and meals across care settings. Demonstrates the knowledge and skills necessary to participate in assessing, planning and care for providing service to participants according to assigned responsibilities and Northland PACE Program standards.
Primary Responsibilities: Under the limited supervision and in accordance with all applicable federal, state and local laws/regulations and policies, procedures and guidelines:
- Provides physical assistance and attendant care to participants across care settings (center, home, transportation).
- Provides basic personal care assistance and/or supervision (bathing, grooming, toileting, feeding) and assistance with any other Activities of Daily Living (ADLs).
- Provides assistance with Instrumental Activities of Daily Living (IADLs) across care settings.
- Performs homemaking, meal preparation, light cleaning and other household duties, shopping and other chore service for participants.
- Provides assistance with nursing care, restorative therapies, activities and meals across care settings.
- Participates in assessing, planning care for and documenting care for participants as member of interdisciplinary team.
- Knows and adheres to the philosophy and goals of the Northland PACE Program.
- Maintains a quiet and safe environment for participants, visitors and staff.
- Keeps confidentiality of participant records, reports and discussions.
- Participates in Northland PACE Program Committees as requested by Director of Nursing (DON).
- Maintains flexibility in schedule and responds to unexpected emergencies and change in workload in order to fulfill responsibilities.
- Utilizes supplies and equipment economically.
- Informs Director of Nursing of “unusual occurrences.”
- Identifies services delivery problems.
- Is professional in appearance and manner in the clinical area; recognizes own limits and seeks help and guidance from the Director of Nursing as appropriate; serves as a role model for students and staff members.
- May serve as a role model for new trainees.
- Pursues professional growth through the continuing education process.
- Maintains certification and pursues professional growth through continuing education process.
Education/Experience: Completion of a CNA training program approved by the State of ND or approved equivalency test. Alternate training includes completion of a home health aide program or a certified nurse’s aide program. Some experience as a homemaker, personal care aide, home health aide or nursing assistant is desirable.
Licensure, registry or certification required: Certified Nursing Assistant in ND. Current, valid North Dakota driver’s license.
Special Training/Skills: Interdisciplinary teamwork, ability to work effectively with culturally, economically and educationally diverse populations, and form positive interpersonal relations in dealing with a wide range of staff and participants is essential. Ability to assist with personal care at home and in center, including transportation. Initiative and resourcefulness. Use of discretion, tact, patience and good judgement. Empathetic and compassionate attitude in care for the elderly. Ability to understand and carry out non-technical verbal and written directions, record messages, and keep records. Ability to get along well with others and gain the cooperation of the participants. Able to hand difficult situations in a professional manner. Routine work involving the personal care and program activities of the participants. Proven record of excellent attendance.
Physical Demands: Must be able to see, hear, and verbally communicate. Must be able to move about freely and possess manual dexterity and visual acuity. Moderate amount of walking. Must be able to move about freely, lift and turn participants.
Working Conditions: Primarily inside, well-lit, climate controlled environment; however, will require travel in various weather elements (rain, snow, cold, warm temperatures etc.). Frequent travel for home visits. Must have reliable transportation. Potential exposure to infectious diseases and environmental hazards.
