Job Description

Assists the professional nursing team members by performing certified nursing assistant duties according to Pinnacle Living policies and procedures. Provides encouragement and assistance designed to motivate residents to their maximum level of independence.

RESPONSIBILITIES

• Provide resident services including personal hygiene, grooming, dressing, nourishment and fluids, skin care, turning and positioning for residents with limited mobility every two hours.
• Assisting residents in and out of bed, walking with residents according to plan of care, and assisting residents to the bathroom as needed.
• Change bed linens at least twice per week and as needed.
• Document care provided in medical record.
• Responds to call lights promptly.
• Assists residents to social and dining events and encourages their participation.
• Provides emotional support, comfort, and safety to all residents.
• Immediately reports any accidents and unusual occurrences to the Charge Nurse.
• Keeps the environment neat, clean, and safe.
• Reports faulty equipment and safety hazards to the nurse in charge immediately.
• Observes and reports all changes in the resident’s condition immediately to the Charge Nurse
• Attends in-service education programs, team conferences, Care Plan/Individualized Service Plan meetings as scheduled, and completes Relias/Learning in-services Completes weights and vitals per plan of care. ..
• Participates in updating residents’ plan of care.
• Upholds the resident Bill of Rights.
• Assists residents with the dining experience. Ensuring residents receive meals and provide assistance with eating when needed.
• If assigned to Assisted Living, additional duties include:
  • Receive daily report from Supervisor regarding special Health Services concerns/needs for residents.
  • . Assisting with activities of daily living to assigned residents.
  • Assists residents to and from the main dining room and to appointments.
  • Monitors for safety in apartments.
  • Respond to any emergency in resident apartments and report to Supervisor.
  • Reports to Supervisor when resident assignments are complete.:
• If assigned to Memory Support, additional duties include
  • Assist resident one-on-one with life enrichment programming in accordance with their individualized service plan.
  • Observes residents’ behavior patterns and reports all changes/differences to Charge Nurse for Individualized Service Plan revisions.
  • Provides companionship for residents as assigned by the charge nurse. Perform other duties as required to ensure the well-being and satisfaction of the resident and their family.
  • Related work as required by Supervisor.
• All other duties as assigned.

QUALIFICATIONS

• Basic conceptual knowledge of theories, practices, and procedures within nursing assistance, typically acquired through certified program.
• Basic verbal and written communication skills required.
• Demonstrates working knowledge of common medical processes and treatments.

EDUCATION, EXPERIENCE, CERTIFICATION AND LICENSURE REQUIREMENTS

• Graduate from an approved Nursing Assistant Program.
• Certified as a Certified Geriatric Nursing Assistant in Virginia.
• CPR/AED certification required.
• 0-2 years of experience preferred.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

• May include frequent lifting (up to 50lbs).
• Extensive walking, bending, stooping, standing.
• Ability to work in a fast-paced environment with multiple tasks.
• Ability to work inside and outside to effectively make rounds daily throughout the facility and to respond to problems and emergencies.
• Universal staffing, holiday, inclement weather, and a flexible work schedule are required.
