Requirements
- Licensed LMSW in Texas or RN license in Texas 2.) BLS required for RN
Must have current licensure/certifications upon hire. New Hires will have 60 days to acquire the required BLS.
Summary
Responsible for ensuring patients are timely and effectively transitioned to appropriate levels of care. Actively participates in daily IDT huddle and identifies patients that are appropriate for transition needs intervention. Reviews census/consults and conducts chart assessments daily for all assigned patients. Collaborates with interdisciplinary team to identify high risk patients who may need a consult for discharge planning/intervention, compiles with all documentation requirements. Communicates discharge plan with patient/responsible party and IDT team. Identifies community resources/needs, facilitates appropriate referrals as needed (acute and non-acute), actively communicates with all appropriate post-acute providers throughout patent stay. Serves as point of contact for all identified stakeholders. Serves as context expert regarding payor information for post-acute care needs. Educates interdisciplinary team and patients/caregivers regarding payor requirements and/or barriers as pertaining to discharge needs/placement. Communicates with payors as appropriate and coordinates appeals/peer to peer conversations when applicable. Escalates issues to appropriate level of leadership and coordinates mitigation activates as needed. Has working knowledge and can facilitate the formulation of Advance Directives. Participates in Joint Commission readiness activities. Participates in department meetings and stays current on continuing education requirements both internally and for appropriate level of licensure. Maintains positive working relationship with staff and IDT. Works in accordance with hospital policies and mission and vision of the organization. Makes reports as necessary and required as a mandatory reporter for adult and child abuse/neglect. Ability to work independently and as a part of a team, must possess proficient problem solving and communication skills and work in a fast-paced environment. Basic computer skills and office equipment and the ability to use the EMR software system and communicate effectively in both written and oral form. Possess the ability and desire to work with all individuals. Performs other job-related duties as assigned.

REPORTING RELATIONSHIPS: Operationally, reports to the Director of Case Management/Social Services
OSHA TASK CATEGORY THREE: There is no anticipated risk of exposure to mucous membranes or skin contact with blood, body fluids, or tissues, or a potential for spills or splashed during the performance of the job. Universal precautions must be observed, and protective devices worn by the employee when performing a task that is likely to cause exposure.
