Social Worker (LSW or LMSW) - Case Management

Position Summary
This position is responsible for identifying and evaluating social and/or physical factors which may either impede full recovery from illness/surgery, or prevent the patient from maintaining a state of well-being. Communicates these factors to the healthcare team and performs interventions as indicated. Assists patient and their families with procurement of community resources as needed. The Unit-based social worker, in conjunction with the Unit –Based Case Manager and the healthcare team, is responsible for arranging all post-discharge care needs. This position has exposure to body fluids and communicable diseases. Majority of time is spent sitting (up to 60% of work time) with frequent standing and/or walking required (up to 40% of work time). Services a variety of populations including: neonates, pediatrics, adolescents, adults and geriatrics.

Licenses/Certifications Required
Bachelor’s or Master's degree in Social Work, Licensed Social Worker (LSW or LMSW), licensed by Arkansas Social Work Licensing Board

Specific Educational and Skill Competencies Required
Knowledge of social work standards of practice and ethics. Experience and knowledge of community resources. Strong interpersonal and communication skills with an ability to work within the team concept. Possesses understanding and ability to fulfill role of social worker in acute hospital setting. Able to perform above duties in crisis situations. Understanding of federal, state, and private payer reimbursement guidelines.
