Virginia Health Services is hiring a Full Time, Care Transition Liaison for Virginia Health Home Care in Virginia Beach, VA!
The Care Transition Liaison performs duties for the VHS Health System under the direction of the Executive Director of VHH/VHHC.
JOB SUMMARY
Responsible for creating an exceptional customer experience through the timely and successful coordination and delivery of appropriate services to our customers. Ensures continuity of care by facilitating open communication between all involved in patient care. Assists customers with transitions between levels of care in collaboration with other health care professionals. Assists with organizing services across service lines, between people, to affect optimal patient outcomes, patient satisfaction, Builds and maintains positive relationships with community leadership, health care providers, community resources, and other stakeholders to develop programs in the community and identify business development opportunities
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Promote the mission, vision and values of the organization
- Facilitates consistent service delivery of the VHS continuum including the highest quality care for the patients and families.
- Participates in resident need identification processes including, but not limited to participating in community patient review meetings, is accountable for completing the referral process including communication to the resident, family, staff, and other professionals as appropriate.
- Provides follow-up on resident hospitalizations and contacts residents returning from the hospital within 24 hours
- Participates in regular meetings with customers to review their experience, quality of care delivery, and program growth.
- Serves as an educational resource to the community regarding healthcare services.
- Coordinates available resources for patients and families.
- Markets VHS to any appropriate audience. including building relationships with referral sources, conducting physician visits, supporting marketing functions, engaging in community networking, and meeting with discharge planners as needed.
- Develops effective rapport with physicians, community associates, discharge planners, and other referral sources and regularly visits them.
- Participates in community VHS program development
- Develops and delivers health educational services.
- Runs all necessary reports to operate the business. Demonstrates the ability to analyze the data, think critically through problems, and solve identified issues to achieve the business objectives.
- Leads new initiatives and is responsible for the successful implementation of programs.
- Perform other duties as assigned
QUALIFICATIONS AND REQUIREMENTS:

- Bachelor's degree preferred
- Knowledge of utilization management principles, home health care, hospice, skilled nursing facilities/long term care, assisted living, independent living and durable medical equipment is highly desirable
- Knowledge of nursing services and insurance coverage preferred
- Strong organizational, interpersonal skills, ability to manage multiple and simultaneous responsibilities and to prioritize of work
- Ability to maintain confidentiality of all medical, financial, and legal information
- Ability to handle difficult situations involving patients, physicians, or others in a professional manner
