Care Manager, Behaviorial Health Job at South County Hospital
The Behavioral Health Care Manager works collaboratively with other members of Community Health to support the behavioral health needs of the patients served. This position and the other team members function as extensions of the primary care practices, working as part of a multidisciplinary team within the practices and the surrounding community. The primary contacts for the BHCM are the Nurse Care Managers of the primary care practices, case managers within the hospital, home health colleagues and behavioral health workers within the practice/community. The BHCM provides home, community and office based behavioral health screenings, assessment, referral and care coordination for medically complex patients who require behavioral health services, including mental health and substance use. In addition, this position provides short term supportive counseling utilizing evidence based strategies, provides patient education, supports treatment adherence, and takes action when non-adherence occurs or symptoms worsen using motivational and health coaching techniques as indicated. This position acts as a liaison between CHT patients and external behavioral health resources, supporting care plan needs.
Reports to: Leader, Social Work Team
Minimum Qualifications:
- Master of Social Work with a minimum of two years direct clinical experience working with adult and/or families providing mental health and substance use counseling
- Three to five years of experience working in at least one of the following areas: community health setting, integrated behavioral health, acute care services or chronic care management program.
- Experience working with primary care providers coordinating care and providing care management services across the healthcare continuum required.
- The ability to travel to various locations in the state, typically via car is required.
- Proficiency in the use of computers, including but not limited to Microsoft Office Suite and EMR software.
Certification and Licensure:
- RI State Licensed Independent Clinical Social Worker (LICSW), required, eligible within 6 months of employment
