Care Coordinator/Case Manager – Drug Court

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Drug Court Care Coordinator/Case Manager.

The Drug Court Care Coordinator is responsible for providing services to participants enrolled in the Merrimack County drug court program. As a member of a multidisciplinary team, the case manager will work closely with the drug court coordinator and the drug court judge.

Responsibilities:

  • Provide comprehensive clinical case management services to all participants including intake assessment, benefit assessment, goal setting, long-term care plan development, weekly case plan development, program monitoring, individual money management, participant education, advocacy, and referrals.
  • Maintain contact with drug court participants to support his/her attendance at interviews and programs.
  • Conduct orientation meetings with new participants that include the program contract, program rules, and expectations of participating.
  • Establish a treatment program for and with each participant by setting schedules and routines, and arrange transportation and escort.
  • Communicate participant progress by participating in weekly interdisciplinary meetings, treatment team meetings, and courtroom testimony as needed.
  • Connect participants with resources, assisting participants in pursuing the best possible options.
  • Assist participants with job training and employment searches; work with area employers to help place participants in appropriate jobs.
  • Assist participants with public assistance programs, financial education, and housing procurement.
  • Assist participants with accessing health and mental health care and support groups
  • Prepare participant discharge by reviewing discharge plans; coordinating discharge and post-discharge requirements.
  • Document all participant-related information on a daily basis.
  • Participate in staff meetings, staff training, and regular supervision.
  • Maintain compliance with federal, state, and local regulations.
  • Compile data for reporting as requested.

Requirements:

  • Bachelor’s degree (Psych, Education, Human Services, Sociology) from an accredited four-year college or university is preferred.
  • Twelve (12) hours of continuing education and training in substance abuse and criminal justice issues is preferred.
  • Valid driver’s license, personal vehicle, and auto insurance or access to a reasonable alternative are required.
  • Experience with a diverse participant population is preferred
  • Familiarity with Alcoholics Anonymous (AA) in conjunction with treatment programs and with the 12-Step philosophy as well as other non-traditional self-help treatment programs
  • Knowledge of Evidence-Based Practices (EBP), Addiction Severity Index
  • CRSW or willingness to pursue is required.
