Who We Need

This position is located at our West Valley Integrated Health Clinic in Phoenix AZ.

The Recovery Navigator (Coach), as a person who is the living evidence of recovery, demonstrates the possibility of recovery through sharing of personal experiences to inspire hope and empower people to succeed in their personal goals. The position works collaboratively with an assigned caseload and with other members of the team to assist people in the development of their Individual Service Recovery Plan (ISRP) that is tailored to person’s needs and preferences and to ensure that all services prescribed in the ISRP are implemented fully and timely. Demonstrates a strength-based approach in interacting with people by fostering skills in self-management. Draws on the individual’s personal strengths and natural supports and empowers the person to be the leader of his/her treatment partnership with the professional mental health providers of care.

MINIMUM QUALIFICATIONS:

  • Requires a High School Diploma/GED and a Certificate from a state approved Peer Support Training Program.
  • A minimum of one (1) to three (3) years’ experience in a behavioral health setting, required.
  • An individual who is, or has been, a recipient of behavioral health services (self-disclosure).
  • Satisfactory work records as determined by the People 1st
  • Successfully completes a pre-employment drug screening.
  • Ability to obtain and maintain a Level One Fingerprint Clearance Card and any other required background checks.
  • Ability to meet Copa’s driver authorized driving requirements, including obtaining an AZ Driver’s license upon hire, 39-month MVR, proof of insurance and vehicle registration, if applicable.

JOB SUMMARY (Including, but not limited to)

  • Participates in morning meetings, staffing and other assigned meetings with participants, family members and collateral agencies
  • Partners with Connective Care Coordinator to provide coverage support when needed or assigned.
  • Works with an assigned caseload of participants to develop and achieve specific ISRP (Individual Service Recovery Plan) goals.
  • Assists in identifying goals in the areas of living, learning, working, wellness, and self-advocacy.
  • Assists in identifying and maintaining benefits for which the participant is eligible.
  • Assists in building healthy social and recreational skills in the community.
  • Provides transportation, sets appointments.
  • Provides coaching through periods of frustration, and identifying natural and family-based supports and community resources
  • Meets with participants in the Arizona State Hospital (ASH)
  • Conducts hospital discharge follow-up.
  • Participates in outreach, engagement, and re-engagement
