Deblin Health Concepts & Associates, Inc. (DHC) is a for-profit mental health service agency providing a wide array of services in Austin, Dallas, Houston, San Antonio, Beaumont, Cleveland, Humble, TX service areas. Mental health counseling, intensive case management, skills training, psychosocial rehab, acute day programs, geriatric counseling, in-home therapy, tele-counseling, hospital discharge planning/liaison services are among some of the services provided.
Responsibilities:
The Case Manager is responsible for the implementation of plans in the direction of the Case Manager Supervisor for Intensive Case Management (ICM) to help each client achieve optimal stability and functioning. He/she familiarizes him/herself with priority client cases, executes tasks in a timely manner, develops resources and establishes key contacts in the database for immediate access of resources to serve the needs of the client. Case Manager will implement agency operations, policies, and procedures and will ensure that the lines of communication between Administration, the field (therapists, and family members, community providers) and the client remain open.

Minimum Qualifications:
- Masters Level Degree
- LMSW
- Valid Drivers License
- An understanding and appreciation of diversity.
- Demonstrate an ability to work on a team.
- Computer literate/some computer knowledge.
- Experience in providing mental health services, preferable and out-patient settings.
- Priority for candidates that will work specifically with children
