General Purpose
Under direction from the Behavioral Health Manager, provide care coordination services to patients who are most at risk for health deterioration, Social Determinates of Health, sentinel events and/or poor outcomes. Ensure the PCP (primary care provider) team and the Behavioral Health Provider (BHP) maintains a central role in coordinating and managing the care of vulnerable patients and that patients receive comprehensive care across multiple health settings and providers; utilize the nursing process to ensure continuity and coordination of care across transitions of care for all patients, with special attention to patients with complex medical, behavioral and social needs. Support pursuit of MFHC Quadruple Aim of improved patient outcomes, enhanced population health, staff satisfaction and controlling cost.

Job Qualifications*
a. High school diploma required
b. Must be bi-lingual- Spanish English
c. Must have 1 year of medical or behavioral health experience.
d. Experience working with Substance Abuse issues preferred.
i. Preferably a CAC I or higher.
