Responsibilities:
This opportunity may require full Covid-19 vaccination and/or submit the duly documented exemption. People with exemption are subject to weekly COVID testing to maintain employment.
- Communicates with community providers about the consumer's treatment needs and provides suitable care plans for individuals with unique needs.
- Follow-up via phone or community outreach, as well as crisis intervention if needed.
- Educates the patient about their disease and other health issues that are relevant to their plan of care.
- Collaborates with other professionals to improve treatment outcomes and care overall.
- Conducts research, reviews, and maintains data on treatment, care, and/or related services.
- Bachelor’s Degree in a Behavioral Health field; Master’s Degree preferred.
- Minimum 2 years of experience in Clinical, Behavioral or Managed Care field preferred.
- Must have a valid clinical license.
- Personal computer experience should include working with Microsoft Office programs, at intermediate level minimum.
- Bilingual (Spanish and English).
- Strong verbal and written communication skills.
- Must have a record of accomplishment of producing work that is highly accurate, demonstrates attention to detail, and reflects well on the organization.
