Hearth, Inc. has an immediate opening for an Assisted Living Director at its Assisted Living Facility at Ruggles.

The Assisted Living Director manages the overall operations of the Assisted Living Facility for Hearth. In the role, the Director hires, trains, and supervises staff, works as a liaison with the on-site property manager. In addition, the Assisted Living Director provides case management/stabilization to a limited number of residential clients.

The Assisted Living Director directly supervises the Resource Specialist, Front Desk/Concierge Staff and Kitchen and Housekeeping Staff. This position reports to the Chief Program Officer at Hearth.

Major Functions:

  • Oversee the smooth day-to-day operations of facility
  • Select staff supervision
  • Case management of select residents including information and referral to community resources and crisis management
  • Coordination of various site activities including but not limited to volunteer coordination, meal program and community engagement
  • Residence liaison to onsite Property Management company

Duties and Responsibilities:

  • Administrative/Fiscal
  • Work as residence liaison with Property Management team.
  • Oversee the food program at site.
  • Work with pertinent administrative staff to oversee and facilitate, applicable certifications and/or licensing processes, audits and site visits.
  • Assist in establishing policies and procedures for smooth facility operation, including but not limited to: fire, safety, emergencies related to Mass MAP/disaster response, emergency medical situations, mental health and risk management procedures.
  • Participate on HEARTH, Inc. management team; attend regular management meetings and participate in various management working groups.
  • Develop, implement, and monitor program annual plan and participate in yearly program evaluation.
  • Attend pertinent outside agency meetings as needed.

Personnel/Supervisory

  • Recruit, hire, train, supervise and oversee scheduling of select regularly scheduled and relief staff.
  • Participate in regular clinical team meetings to review plan of care and to optimize quality of life for each tenant.
  • Lead regular program staff meetings to clarify clinical, administrative, and program issues and goals and to review pertinent policy, procedure, and case management issues and/or information.
  • Oversee and review yearly staff evaluations as required by regulation.
  • Identify, communicate, and facilitate internal/external staff opportunities for continuing education and professional support through workshops, trainings, and conferences etc.
  • Oversee appropriate program coverage at all times and arrange for back-up coverage when necessary.
  • Provide on-call back-up coverage as necessary.
  • Coordinate and manage site specific volunteer activities with Assistant Director of Volunteer Management.

Tenant Screening and Assessment

  • Participate in tenant selection and screening process as needed.
  • Oversee case management for all residents including (a) collaborating and consulting with individual service providers, (b) creating, updating and monitoring compliance with service plans and contracts (c) working with pertinent contract providers to ensure the provision of appropriate case management for their clients in residence.
  • Develop and maintain strong links to community providers and elder service providers with attention to continuity of care and community engagement.
  • Lead regular tenant meetings to review critical tenant issues and fire and safety procedures.
  • Maintain accurate records and documentation consistent with contract and agency guidelines (e.g. DMH, GAFC, SCO).
  • Plan/attend case management meetings, multi-disciplinary and family meetings when indicated.
  • Participates with clinical team in crisis management as needed.

Health Screening and Physical Requirements:

  • Annual Influenzas (Flu) vaccination
  • Be fully vaccinated for COVID-19 and any recommended booster shots
  • Provide a pre-employment and annual physical exam

OSHA Rating: Risk of exposure to blood-borne and airborne pathogens.

Pay: $53,000.00 - $57,000.00 per year
