Description

The Renaissance School of Medicine (RSOM) at Stony Brook is recruiting an Assistant Dean for Clinical Integration/Chief of Population Health (ADCI/CPH). This role will focus on improving quality and reducing overall medical expense while optimizing care delivery models for Stony Brook Medicine. The ADCI/CPH will lead the design and implementation of key strategies focused on innovative payment models with both payors and providers. The ADCI/CPH will provide oversight of both Stony Brook Medicine’s Clinically Integrated Network (SBCN, LLC) and the Stony Brook Medicine Accountable Care Organization (Stony Brook ACO, LLC).

The ACDI/CPH reports to the Vice Dean for Clinical Affairs / Vice President for Clinical Services. Working closely with the Executive Director of the CIN/ACO - they will have oversight of CIN/ACOs teams focused on quality/analytics; care management; physician transformation; and disease management. In addition, they will lead SBM’s efforts with their Post-Acute Care Collaborative (PACC) to align population health initiatives across the continuum of care.

The selected candidate will have a faculty appointment in a RSOM department based on their area of expertise.

Stony Brook Medicine consists of 4 Hospitals – Stony Brook University Hospital, Stony Brook Children’s Hospital, Stony Brook Southampton Hospital and Stony Brook Eastern Long Island Hospital. The Physician network comprises the 1400 providers across the Full-Time Faculty of the RSOM and 2 employed physician networks – Stony Brook Community Medical and Meetinghouse Lane. These providers span across Suffolk County, a population of 1.6 million people. Stony Brook University Hospital is the only Academic Tertiary Care Center and Stony Brook Children’s is the only Children’s hospital in Suffolk County. Stony Brook Southampton Hospital and Stony Brook Eastern Long Island serves a diverse population in the heart of Long Island’s east end.

Duties/Responsibilities:

  • In conjunction with Stony Brook Medicine Leadership, specifically the Executive Director of the CIN, provides operational support for Stony Brook Medicine’s value-based care strategies, goals and objectives.
  • Works closely with leaders from Stony Brook Medicine and other key partners to support and participate in the governance of Stony Brook Medicine’s population health programs and initiatives.
  • Collaborates with leaders from Stony Brook Medicine and other key partners to develop effective population health strategies and prioritize the population health work, including choosing appropriate key performance indicators.
  • Recommends strategies and at-risk models to support Stony Brook Medicine’s strategic plan.
  • Responsible for clinical operations that maximize Stony Brook Medicine’s performance on value-based purchasing metrics while minimizing financial penalties associated with those same metrics.
  • Supports the value-based care contracting teams, ensuring opportunities are aligned with success in value-based care and population health management.
  • Develops and leads initiatives to partner and engage our underserved communities and high-risk populations to improve access to health care resources.
  • Works closely with the CIN Ambulatory Care Management team to provide guidance and oversight for ongoing programs and activities.
  • Provides oversight to the CIN clinical outcomes workgroups including the appointment of clinical leads for as well as establishing goals and objectives.
  • Provides ongoing support for the operational aspects of system and regional value-based care initiatives.
  • Partners with Stony Brook Medicine leaders to grow and effectively manage Stony Brook Medicine’s attributed value-based lives within the accountable care organization.
  • Leads efforts to broaden reach and effectiveness of Stony Brook Medicine’s health initiatives through development and support of new delivery models.
  • Ensures that population health strategies and initiatives are aligned with Stony Brook Medicine’s financial goals.
  • Provides business and operational support for partnerships and third-party relationships to achieve SBM’s population health strategies and goals.
  • Works as a liaison with Stony Brook Medicine IT to leverage the electronic medical record to achieve metrics for population health strategies.
  • Provides oversight of the quality analytics team for Stony Brook Medicine’s Clinically Integrated Network.
  • Leads the Practice Transformation team and collaborates with Disease Management Teams to develop and advance population health initiatives.
  • Provides oversight of the Post-Acute Care Collaborative to align population health initiatives across the continuum of care.
  • Creates a culture that bridges community and population health activities.
  • In addition to the Executive Director for the CIN/ACO/IPA, other key collaborators include, but are not limited to, Chief Quality Officer (CQO), Chief Medical Officer (CMO), Chief Strategy Officer, Director of Care Management, et al.

Qualifications

REQUIRED QUALIFICATIONS:

  • MD, PhD or equivalent degree.
  • Ten years of healthcare related operational experience with five years in a leadership role.
  • Experience in an Academic Medical Center or other complex healthcare organization.
  • Strong strategic thinking skills with experience articulating a clear vision and developing an implementation plan that builds support among a diverse set of constituents that may have competing objectives.
  • Highly effective verbal, written and interpersonal skills.

PREFERRED QUALIFICATIONS:

  • Master’s degree in Business or Healthcare related field or Equivalent Experience in Population Health.
  • Experience with value-based care and population healthcare operations across the care continuum landscape to include acute, outpatient, ambulatory, post-acute and care at home.
  • Experience with different lines of business, particularly as it relates to value-based care; commercial, Medicare Advantage, managed Medicaid, direct-to-employer, self-funded, etc.
  • Experience with value-based payer contracting and reimbursement models such as pay for performance, shared savings, shared risk, capitation, and percent of premium.
  • Understanding of the application of analytic insights pertaining to population health strategies and operations.
  • Experience with population health performance metrics and supporting tools.
  • Experience with employed medical groups and Clinically Integrated Networks as part of population health management.
