Airport Outreach Case Manager Job at HOPE Atlanta Programs of Travelers Aid
The Outreach Case Manger will be responsible for providing outreach, case management services and referral services to chronically homeless individuals within the city of Atlanta, specifically at Hartsfield-Jackson Airport. This is a Part-time position.
1. Provide connection to coordinated entry services to include performing mobile assessments where clients are located, referring/connecting clients to immediate shelter and providing ongoing navigation to assist that client with obtaining permanent housing.
2. Connect participants to community resources for treatment, life skills programs, and employment assistance.
3. Develop Individuals Service Plan (ISP) for each client and documenting in the ClientTrack HMIS system, as well as make referrals to other service providers to support improved access to mainstream benefits.
4. Provide weekly report to Outreach Coordinator of all clients engaged, enrolled, housed and/or connected to resources and services.
5. Keep detailed records of the areas of the city where people staying outside have been engaged.
6. Complete ClientTrack Eligibility for each participant entering case notes within 48 hours of engagement.
8. Maintain case files for each and every engaged individual.
9. Ability to approach and engage potential clients professionally.
Knowledge, Skills, Abilities
- Experienced with Outreach programs
- Knowledgeable of mental illness, addiction, and PTSD
- Proficient in Microsoft Office products, ClientTrack, and database systems
- Flexibility and multi-tasking skills
- Excellent verbal, written, and communication skills
- Valid Georgia driver's license with good driving record.
- Utilize good judgement and emotional intelligence in engagement with the public.
Education, Experience
- Bachelor's Degree in Social Work, Psychology or related field, strongly preferred
- Experience in outreach, homeless population, and/or related vulnerable communities preferred.
