Why does the job exist?
The purpose of this position is to identify, coordinate, review, organize, and evaluate the daily living needs of individuals calling the Aging & Disability Resource Center (ADRC) with Aging and Long Term Services Department. The position will provide assistance to those who do not have the ability to resolve problems or access services on their own. The main purpose is to help consumers identify and understand their needs and to assist them in making informed decisions about appropriate long-term service and support choices in the context of their personal needs, preferences, values and individual circumstances. Resource Options Coordinators also involves assisting people in understanding their health care and other benefits through SHIP, as well as helping middle aged individuals to plan ahead for their long-term care through the use of private insurance, home equity mortgage and alternative living arrangements.
How does it get done?
Provide over the phone and walk-in counseling in care coordination.
Conduct an assessment to address and identify the various needs of the clients. Provides assistance in complex cases by providing information, engaging in identifying clients needs, and offering appropriate information in order to increase access to social service programs.
Provide individual client counseling regarding Medicare, Medicaid, health and long term care insurance, public benefits and appropriate community programs and services. Incumbent will review eligibility determinations for such social service programs.
Provide information and assistance with two (2) or more client needs, determine the actions previously taken, assistance previously provided, and results of the actions and assistance in order to refine the list of possible resources and assistance that may be needed. Identify appropriate available services and supports by providing enough information and referral about each organization to help inquires make informed choices
Complete current client contact records.
Complete the record of the current contact by obtaining additional information, if necessary, and document assessment, client demographic information, outcomes, referrals, client notes and note follow up efforts in ADRC data base.

The ideal candidate must be professional, courteous, civil, respectful and must provide open, honest and thorough communication.
Minimum Qualification
Bachelor's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology and/or Family Studies/Services. Substitutions apply. See Substitution Table below.

Employment Requirements
Must possess and maintain a valid Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification within 90 days of employment.
Occasion travel to regional offices.
Working Conditions
State of New Mexico employees may be impacted by the Public Health Order dated August 12, 2022, which requires facilities licensed or certified by the Centers for Medicare and Medicaid Services ("CMS"), including all hospital types, long-term care facilities, nursing homes, hospice facilities and rehabilitation facilities, as well as assisted living facilities and adult day care settings, to adhere to all COVID-related requirements prescribed by CMS, including, but not limited to masking and staff vaccination.
Must provide proof of being Fully Vaccinated for COVID-19.
Work is performed in an office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and telephone usage.
