About this position
Reviews and coordinates response to Advance Care Planning consult orders at Blaze Health and serves as a resource to Blaze on Advance Care planning and end of life care.
What we’ll expect from you
- Work with members of interdisciplinary team regarding consult expectations.
- Complete ACP Facilitator training and maintain expertise in this area.
- Work with Program Manager to establish best practice policies and procedures.
- Provide facilitated ACP conversations to the most complex ACP consults at Blaze Health.
- Collaborate with other trained facilitators to address other consults at Blaze Health.
- Document using ACP protocol.
- Support ACP Facilitator training as needed
- Provide informal ACP consultation to Community Care Management department and other staff.
- Assist in the development and maintenance of ACP resource materials for Blaze.
- Assists in the development and execution of ACP Community Education classes.
- Accepts other duties as assigned to promote the accomplishment of organizational goals.
- The team member is accountable to demonstrate proficiency for the skills outlined in the appropriate position skills list.
- The team member is accountable to maintain skill proficiency, including improvement where deemed necessary, and upgrading any additional or new skills on the appropriate position skills list.

- Recognizes and complies with legal regulatory, accrediting and procedural requirements related to area of responsibility.
- Understands and follows principles and standards as outlined in North Memorial’s Corporate Code of Conduct.
- Protects customer/team member privacy and only accesses customer/team member related information as needed to perform job duties.
- Reports violations or areas of concern to supervisor or Corporate Compliance Officer via established methods of communication.

Education
- Bachelor's or Master's degree in Social Work, OR
- Associate's or Bachelor's degree in Nursing
Experience, Knowledge, Skills, Abilities
- Two years medical social work or nursing experience required.
- Palliative Care or Hospice experience preferred.
- Advance Care Planning experience strongly preferred.
Knowledge, Skills and Abilities:
- Strong verbal and written communications skills.
- Strong interpersonal skills.
Licensure/Certification(s) Mandatory
- Licensure as Registered Nurse in the State of Minnesota; OR
- Licensure as a Social Worker in the State of Minnesota
- Valid Class D driver's license in the state of residency with an acceptable driving record
