We are currently seeking a caring and resilient Facilitator to join our Transition to Independence Process (TIP) program in our Delaware County region!
The Transition to Independence Process (TIP) model is an evidence informed practice developed for working with youth and young adults aged 16-26 with emotional/behavioral difficulties to: a) engage them in their own futures planning process; b) provide them with developmentally-appropriate, non-stigmatizing, culturally-competent, and appealing services and supports; and c) involve them and their families and other informal key players in a process that prepares and facilitates them in their movement toward greater self-sufficiency and successful achievement of their goals related to relevant transition domains. The TIP Facilitator will also provide advocacy to youth in order to help youth set and achieve recovery goals and will empower youth to increase self-advocacy during their transition to adulthood.
Program Specific Responsibilities:
- Maintain a caseload of at least 8 youth with serious mental illness or emotional disorders.
- Provide case management services within the context of the youth’s culture under the TIP model Guidelines.
- Ensure all youth assigned receive an environmental matrix to identify skill deficits and strengths to establish service plan goals across 5 relevant transition domains: (1) Employment and Career (2) Educational Opportunities (3) Living Situation (4) Personal Effectiveness and Wellbeing (5) Community-Life Functioning
- Provide case management services following a goal and outcome-oriented service plan based on the TIP model. The service plan must be developed within 30 days of admission into the program, reviewed, and updated at least every 6 months.
- Provide services that diffuse or resolve crises in order to prevent unnecessary use of crisis center, inpatient services, or mental health court.
- Support the youth in obtaining formal and informal supports specified in the service plan and building a support network.
- Provide case management services as needed in the community where the youth resides or wherever services would be most appropriately delivered.
- Achieve billable hour requirements per week.
- Provide on call rotation for 24/7 mental health crisis support.
- Perform other related duties as assigned or requested.
Basic Qualifications:
- High school diploma or GED
- 12 credits in coursework related to the Human Services field
- Valid driver’s license and car
- Ability to reflect the culture and understand the values of the Transition Aged Youth population is preferred
Preferred Qualifications:
- Associates or Bachelors degree with major coursework in Human Services or a related field
- Minimum of one year of direct services experience in the human services system
- Demonstrate excellent communication skills
- Ability to learn about community resources in Delaware County and the surrounding area
Compensation and Benefits:
- Starting rate of $18.27 per hour
