We are Custom Home Care, the premier home care agency in Chicago, and we are looking for an energetic nurse manager to join our team. Ideal candidate would have a minimum of 3 years nursing experience and at least 1 year of experience with seniors/geriatrics. Some of the duties will include managing caregivers with their respective clients, client assessments, nursing care management, and discussing CHC services to prospective clients and/or their families. Illinois Registered Nurse (RN) or LPN license is required.

• Valid Illinois Registered Nurse license or LPN
• Authorized to work in the USA
• Experience with the behaviors of dementia strongly recommended
• Two to three professional references
• Submit to state/national criminal background check, TB testing and drug screening
