The Role: Team Lead, Housing First, Hampton Roads

What You'll Do: Reporting directly to the Associate Director, you'll oversee and provide the supportive services our residents need to maintain housing.

• Provide daily oversight of the supportive services provided at multiple sites by the team: case management guidance related to assessing current needs, establishing care plans & include a move on plans, linking to community resources, assisting with applications, helping individuals move, engaging individuals in skill-building activities, providing ongoing and follow-up support to promote housing stability
• Provide case management expertise on assessments, treatment planning, supportive counseling, housing stabilization services, monitoring progress, collaboration with other providers, and care coordination
• Organize, facilitate, and/or coordinate appropriate workforce development, job training, and vocational services to promote employability skills
• Adhere to grant requirements, assist with annual grant reporting documents, and ensure the team's compliance with all required reporting, data entry and quality control in accordance with VSH, HUD, CoC, and HMIS standards
• Assist with outreach and engagement activities to include clients and their families, community providers, property management staff, and medical personnel
• Assist clients with securing and maintaining entitlements and benefits through DSS and other income and disability support resources

What You'll Bring to VSH:

• A Master's degree in human services with 3+ years experience supervising and providing direct services to individuals with low-income, physical disabilities, behavioral health and developmental disorders, and/or history of homelessness OR a Bachelor's degree in human services with 5+ years experience
• At least 1 year of experience supervising/mentoring/training others
• Knowledge of homeless population and appropriate community resources, especially entitlement and housing resources
• SOAR certification and experience strongly preferred
• Knowledge of mental illness, substance abuse disorders, community resources, medication management, and recovery concepts
• Certification in CPR and First Aid preferred
