THE POSITION
Service Coordination is a case management service that assists individuals eligible for Medicaid in gaining and coordinating access to necessary care and services appropriate to the needs of the individual.
POSITION EXPERIENCE & EDUCATION REQUIREMENTS
- Must be at least 18 years of age
- Bachelor's Degree in a Human Service Field
- Must have 12 months experience working with individuals with developmental disabilities
- Have their own reliable transportation, a valid Driver's License and car insurance
- Must be able to pass State and Federal Background Check through the Department of Health and Welfare
PREFERRED QUALIFICATIONS
- Current First Aid/CPR Certification
ESSENTIAL FUNCTIONS
- Develop, implement, and revise participant plans, as necessary
- Collaborate with participant, families, and other agencies to ensure services are provided
- Maintain contact with families and participants, both via phone and face to face as required
- Coordinate with the participant to obtain crisis resolution, as needed
- Adhere to the rules of all governing agencies, including IDAPA
- Provide assistance to people receiving support through the implementation of the RISE mission, vision, and values
- Demonstrate respect and promote choice
- Promote the rights of people receiving support and contribute to an environment that is free of abuse, neglect and or exploitation
- Support people in achieving outcomes identified in the persons plan; this includes keeping required documentation and any prescribed or related task(s)
