Street Outreach Peer Navigator

GENERAL DESCRIPTION

The Street Outreach Peer Navigator is primarily responsible for providing case management, Housing Navigation, support, advocacy and resources to individuals and households experiencing literal homelessness. Peer navigators are expected to provide consistent engagement with client centered and trauma informed lens meeting the need where the client. is emotionally and . Generally these needs involve locating food, shelter, employment and obtaining medical, mental health and substance use resources. Peer Navigators are required to enter data into HMI5 system and mapping technology.

DETAILED JOB DESCRIPTION

Responsibilities:

Engage with Clients and maintain a formal Caseload which will include:

• Utilize mapping software
• Routinely complete HMIS enrollments and enter client data and case notes into NC Homeless Management Information System (HMIS) in accordance with the program
• Complete Coordinated assessments refer to available resources.
• Assists individuals and families with the documentation needed for Housing resources: NCID, Social Security Card, disability verification, homeless verification, and income verification.
• Provide Street Outreach to individuals and families living in places NOT meant for human habitation in Mecklenburg County
• Follow up with community referrals.
• Most participate in Mecklenburg County's annual Point in Time Count in a lead position
• Transport clients to appointments and resources as needed
• Collaborate with Licensed Clinician on an outreach team to coordinate services for individuals needing assistance with mental health and substance use services
• Act as "Eves and Ears" for our unsheltered community as a liaison between city and county agencies and the unsheltered community.

WORK EXPERIENCE REQUIREMENTS

Must have relevant experience working with individuals experiencing homelessness with an attention to substance use and mental health issues.

Job Type: Full-time

Pay: $18.00 - $20.00 per hour
