Position Description:
Under the direction of the Senior Director for Population Health Quality Performance , the Senior Program Manager will provide support to the clinical and operational leadership of Primary Care to assist in their achievement of established targets on priority performance measures. The Senior Program Manager will provide planning, development, monitoring and project management of the ACO and Medicaid Waiver deliverables and a wide range of administrative and operational initiatives.

This role will consist of collaborative work with Primary Care leadership and clinical teams, as well as departments throughout CHA (Accountable Care Organization, IT, Quality, Performance Improvement, Government Affairs) to improve systems of care in order to support the achievement of ACO and Waiver metrics and other reporting and performance deliverables. The Senior Program Manager will partner closely with performance improvement staff assigned by the Quality and Safety Department to Primary Care to ensure alignment in focusing on the designated quality improvement priorities.

Under the direction of the Senior Director of Population Health Quality Performance, the Senior Program Manager will assist Primary care leadership in planning, implementing, evaluating, and continuously improving many simultaneous projects to achieve strategic priorities for Primary Care. These initiatives will include a wide range of activities related to strategic planning, practice transformation, change management, workforce engagement, data systems and quality/process improvement.

Specific Duties & Responsibilities:
1. Collaborate with the Sr. Director for Population Health Quality Performance , Primary Care (PC) leadership, as well as departments throughout CHA to create project plans to improve systems of care within Primary Care in order to achieve ACO and Medicaid Waiver reporting and performance deliverables. These deliverables are necessary to satisfy all value-based and pay-for-performance contracts including: developing quality measures and analytic tools for performance management; producing reports for the Executive Office of Health & Human Services; in conjunction with the PC Performance Improvement (PI) Consultant and PC leadership, establish improved clinical and operational workflows; create and manage complex, multi-year work plans to support the organization, prioritization, and monitoring of performance improvement to meet metric targets.
2. Develop detailed reports on deliverables and quality metric performance for the ACO and Medicaid Waiver contracts. Complete timely submission of reports on care transformation deliverables and quality performance, both to internal leaders for monitoring and performance improvement, as well as external audiences in order to claim contingent incentive payments related to the Medicaid Waiver and the ACO.
3. Partner with the PC PI Consultant and PC leadership to develop and execute targeted initiatives to improve performance on key quality measures for Primary Care. The Senior Program Manager may be asked to participate in PI projects as needed.
4. Facilitate and/or lead the development of quality metrics. Complete the validation and ongoing monitoring of quality metrics included in the ACO and Waiver programs. This will include assisting with the development of Epic enhancements, analytical reports, tools and other technologies.
5. Promote a thriving PC Population Health Committee structure by engaging clinical and administrative leaders and staff through buy-in, optimism and collaboration amongst the group.
6. With guidance from the PC PI Consultant and PC leaders, assist in the development and presentation of training materials for PC staff and providers to understand new performance improvement initiatives and workflows.
7. Facilitate or participate as appropriate in interdepartmental committees relevant to the quality performance improvement priorities.
8. As needed, offer technical assistance to clinical and operational leaders on quality measurement, building improvement capability, population health management, care integration & coordination, EMR optimization, and more. 9. Supervise a Program Manager that provides oversight to Planned Care Coordinators who support quality performance metrics by coordinating outreach to specified patient populations.
10. Performs other related duties as assigned or directed such as the Phytel contract for implementation and oversight.

Qualifications/Requirements (*preferred)
  • BA or BS Degree required, MA or MS Degree preferred, in health care, administration, or research (Public Health, Hospital Administration, Business Administration w/ health care concentration; Social Work, Psychology, Nursing, etc.)
  • Three or more years of experience in program and project management.
  • Demonstrated ability to design, implement, and improve new initiatives (clinical or managerial)
  • Demonstrated skills with quality measurement, performance monitoring and improvement.
  • Knowledge of concepts and frameworks for improvement science, quality improvement.
  • Demonstrated analytical capabilities.
Excellent written and verbal communication skills. Strong presentation skills and ability to speak comfortably with large audiences. Ability to maintain a high-energy, persistent, assertive, diplomatic, and cooperative work style.
