Job Description

Price County is accepting applications for a Certified Social Worker to join our team in the Aging and Disability Unit of the Health and Human Services Department. This position is responsible for providing Adult Protective Services (APS) and Aging and Disability Resource Center (ADRC) duties. Work involves investigation of elder/vulnerable adult abuse, guardianship and protective placement actions including evaluations, court studies and reports, information and assistance, options counseling, functional screening, enrollment and disenrollment counseling. Requires court testimony, use of the court’s electronic filing system, data entry and tracking, and intake duties for the agency.

This is a full-time, regular position with a full benefit package and starting hourly wage range of $24.65 to $26.22 (Grade H) depending on experience. Minimum qualifications include:

  • Bachelor’s Degree in Social Work or another related field that qualifies the individual for certification as a Social Worker
  • Current certification in the state of Wisconsin as a Social Worker or attainment within 12 months of employment
  • Two years post degree experience in a human service setting working with target population preferred
  • The ability to work a flexible schedule
  • Possess a valid Wisconsin driver’s license and access to reliable vehicle insured to county-required limits.


Job Responsibilities

50%
  • Responsibilities of the Aging & Disability Unit (50%)
  • Conducts investigations on referrals of abuse, neglect and exploitation of vulnerable, disabled adults and elder persons
  • Conducts State of Wisconsin Chapter 55 protective placement and Chapter 54/880 guardianship investigations
  • Testifies in Court as needed and appropriate
  • Enters documentation into State reporting system and processes electronic filing through the court systems
  • May complete special assignments as requested by supervisor which require experience (public presentations, serving on a committee, preparing a report)
  • Provides mentoring as requested and required to co-workers
  • Provides training and education to other agencies and general public on Adult Protective Services on issues
  • Perform intake duties
  • Consults with Supervisor on case status

  • Documentation (35%)
  • Carefully, accurately & completely document all client and collateral contacts and all necessary forms in accordance with program regulations and agency policy
  • Prepares appropriate documentation and court reports
  • Maintains case records to include pertinent, current, accurate information and correspondence, reports and other required documents
  • Complete daily time studies in accordance with agency policy

  • Review and approve monthly provider billings and generate payments when necessary
  • Code daily time logs correctly in order to maximize funding sources.
  • Attend unit meetings and Department All Staff meetings
  • Maintain knowledge of procedural, administrative, and program updates and changes
  • Attend required and recommended training

  • Miscellaneous (5%)
  • Appear and testify in court, when necessary.
  • Provide intake duties on a rotational basis.
  • Assist with emergency planning activities as needed

  • Knowledge of and adherence to agency programs, procedures, regulations, policies, and applicable statutes/laws.
  • Knowledge and experience with ADRC, Family Care programs, Nursing Homes, community care facilities, Waiver programs, the target groups of Elderly, Developmentally Disabled, Physically Disabled and Mentally Ill is helpful.
  • Skillful in communicating effectively and developing rapport with others in a non-judgmental manner.

  • Special Requirements:
  • Must produce evidence of meeting or exceeding the minimum automobile liability insurance requirements contained in the Price County Minimum Insurance Ordinance (Currently $100,000 per person or $300,000 per accident for bodily injury, $100,000 per accident or $300,000 combined single limit for property damage, and $100,000 per accident or $300,000 combined Uninsured Motorist), in order to operate a vehicle on agency business.
  • Must be able to pass the Caregiver Background check as defined under Wisconsin Administrative Code HFS 12.
