Job Summary and Qualifications

An interdisciplinary team member who serves as a resource and consultant to the hospital staff and a referral contact for community resources and agencies. Assists as needed to develop appropriate treatment programs for patients and families to address the emotional aspects of an acute illness. Provides a supportive relationship and counseling as indicated in the managing of problems in acceptance of illness, disability, medical recommendations, adjustment to medical setting and treatment, personal and life situations of patient and family, losses (altered body image, loss of accustomed role, death), illness, economic hardship, and the need for institutionalization. Presents programs/workshops for the hospital based on existing needs, Participates in community organizations relevant to the position, develops and maintains good working relationship with outside agencies and assists in developing new community resources needed by patients and families. Assists department by assuming coverage for case management duties when need arises and maintains good working relationship with co-workers, nursing, physicians and other related departments as required. Assists case manager by providing additional direction in finding resources to help patient meet basic needs for post-hospital care when need arises by utilizing resources such as family, friends, church, private and public agencies and through counseling and education, encourages and assets patient and families to make efforts themselves to obtain and use resources. Coordinates and insures the provision of hospital social work services. Provides emotional support by assisting patient/family in emergency ethics or crisis situations.

EDUCATION:

• 
  Bachelors Degree in Behavioral Health Field required, Masters Degree preferred.

EXPERIENCE:

• Minimum of 4 years social work experience in a healthcare setting with experience in discharge planning and utilization review preferred.
