ESSENTIAL FUNCTIONS, QUALIFICATIONS & SKILLS:


Concrete Services to Residents


  • Transportation applications are filled out with-in the indicated deadline.
  • Assessable and responsive to resident’s and/or family’s requests.
  • Phone calls are promptly returned; with-in 24-48 hours.
  • Completion of resident/family requests for services such as: applications for services, arrangements for family visits/camps, etc… with-in a timely manner.

Coordinates and Facilitates Concern Process


  • Refers concerns to appropriate staff for resolution
    • Tracks resolutions and reports responses back to resident with-in five (5) business days.
    • Proactive social services interventions are utilized to work with individuals and their family who are recently admitted and/or have psychosocial needs related to living situation.
    • Investigation of expressed Resident concerns are thorough, timely and complete.
    • On- going support and interventions are provided to residents/families who have expressed concerns.

Advocating for Residents of Inglis House/Residents Rights


  • Provides a consistent forum for helping residents express their needs, concerns and wishes.
  • Speaks on behalf of a resident and will serve as a buffer when necessary.
  • Informs residents of ombudsman services when needed.
  • Schedules and facilitates family meetings as needed.
  • Offers and assists in the completion of Living Wills with-in seventy-two (72) hours of admission.
  • Reports and addresses possible allegations of abuse and/or neglect as they occur.
  • Interviews affected residents when allegations are made.
  • Interviews other residents to help resolving and evaluating this concern as required.
  • Documents findings of reported allegation within two (2) business days.
  • Reviews and documents resident rights with resident on an annual basis.

Documentation


  • Progress notes for behavioral concerns are completed and charted within one (1) business day of resident contact.
  • MDS are completed within two (2) business days of the assessment reference date.
  • Social Work assessments are completed and charted within fourteen (14) days of admission.
  • Living Wills are reviewed for accuracy on a quarterly basis.
  • Quarterly & Annual notes are completed and reviewed for accuracy within the specified time. frame as determined by the supervisor.
  • Social Services interventions for identified psychosocial needs are initiated and followed through in the care plan process.

Behavioral & Mental Health Interventions


  • Complete quarterly assessments and quarterly notes.
  • Attends quarterly Psycho- active/behavioral review team meetings.
  • Attends monthly review meetings with Psychiatrist.
  • Consults with Director of Social Work, Psychologist, and Psychiatrist to develop behavioral plans and contracts as needed.
  • Develops and leads psycho-educational and/or therapeutic groups as needed.
  • Administers specialized interventions to residents of Inglis House.

In addition to the necessary skills and experience to perform the responsibilities outlined above, there are a number of traits that a successful candidate will possess.

  • Required –Undergraduate degree in Social Services or Human Services from a recognized and accredited institution.

5+ years experience working in LTC

  • Experience with discharge planning for Short Term Acute Rehab as well as through Nursing Home Transition process for LTC residents
  • Team oriented
  • Experience with Point Click Care and completing MDS


The Inglis Social Services Coordinator/ Social Worker has the primary responsibility of providing behavioral health services that encourages and supports an environment which is least restrictive to the psychosocial health to a resident of Inglis House. The social worker shall provide concrete services, evaluate mood, monitor behavior, and implement appropriate interventions, as evidence by developing behavioral plans which may include working with resident’s families and their concerns. The social worker will serve as an advocate for the resident as well as a liaison to the facility, family members, and representatives from other entities. All essential job responsibilities will be executed in accordance with Inglis Administrative procedures and compliance/ethics guidelines.
