A DAY IN THE LIFE OF A SUPPORTIVE HOUSING CARE COORDINATOR
In this care coordinator position, you provide outreach and psycho-social rehabilitation services to individuals who struggle to maintain their residences. Following the "housing first" model, you focus on getting individuals a stable place to live before addressing other concerns. Because housing is essential for individuals to be able to work towards other goals, that is always your priority.
You also assist clients with addressing other problems such as safety planning, assessing strengths, goal planning, benefits access, symptom management, and more. Conscientiously, you monitor and evaluate each client's progress towards specific goals around housing, employment, and harm reduction. Additionally, you help to provide skills for behavioral health, medication management, financial management, and activities of daily living. QUALIFICATIONS FOR A SUPPORTIVE HOUSING CARE COORDINATOR
This case management position will meet one or more of the following requirements:
- Bachelor's degree in mental health, social services, or a related field, plus one year of experience relating to or assisting individuals with mental illness
- Certification as a qualified mental health associate (QMHA) through the Mental Health and Addiction Certification Board of Oregon or the ability to meet the criteria to be eligible for certification within 3 months of hire
Additionally, candidates are required to fulfill requirements set by the Laurel Hill Center, including:
- Valid Oregon driver's license
- Willingness to undergo a background check and drug screening upon hire
- Full vaccination against COVID-19 including the booster when eligible

WORK SCHEDULE FOR A SUPPORTIVE HOUSING CARE COORDINATOR
This full-time behavioral health position works four 10-hour days, Sunday - Wednesday. Hours are between 8:00 am - 8:00 pm.
