        SCOPE OF POSITION
       
The RN - Manager, Disease Specific Careassists the Quality Department in overall quality of care issues pertaining to high-risk, disease-specific care. The Manager will be responsible for a reasonable scope of work pertaining to such diseases as congestive heart failure, chronic obstructive pulmonary disease, pneumonia, acute myocardial infarction, stroke, and/or total joint replacement (other specified diseases may be included as needed). The Manager is responsible for assisting with coordination of care from admission through post discharge for the defined populations. The Manager serves as a liaison between the facility, the Medical Staff, Affiliates, Referral Physicians, the Community, and other external health care facilities and health professionals. The Disease Manager works closely with other members of business development, marketing, medical staff services and administration.
 
       
The Manager is responsible for evidence-based protocols and plans of care including physician communication and education. This individual will evaluate patient progress and appropriateness of therapy ordered for patients. The Manager will review patient charts, demonstrate a good rapport with patients while maintaining a professional attitude. Additionaly, this individual is responsible for implementing program changes to ensure program success as patient, physician, or facility needs change and/or any regulations/best practices associated with the program change. The individual must have knowledge of Quality metrics and initiatives and assist care providers in complying with requirements/evidence-based care. This Manager is responsible for communicating, coordinating, and tracking services/care provided within the disease program. This will involve collecting and compiling outcomes data and assisting with implementing changes to processes for quality improvement. The Manager participates in all phases of Quality and Pillar Target compliance measures by reviewing concurrent data and communicating the necessary adjustments to meet target goals.

EDUCATION AND EXPERIENCE
Associate's or Bachelor's degree from an accredited Nursing program. Three years experience in a hospital setting. Knowledge of marketing and public relations with program development and implementation skills experience required.
 
       
Valid and active RN licensure in appropriate state
CPR required
