Resident Care Coordinator Job at Elite Care
The Resident Care Coordinator (RCC) plays an integral leadership role in assisting the Community Manager (CM) in keeping Elite Care’s complex needs house model running smoothly. The RCC and the nurse work as a team to manage resident care, oversee medication management, and respond with professional correspondence and documentation to medical providers, outside agencies, family members, POAs and state agencies. Additionally, the RCC assists with the training, onboarding, and career development of Elite Care team members; manages staffing and related schedules; and works with HR to hire, coach, and promote talent. RCCs must have strong interpersonal and team-building skills and effective communication. The person in this role must be an organized and detailed self-starter with excellent leadership and coaching skills. KEY RESPONSIBILITIES
Training, Onboarding, and Career Development
- Train new Personal Assistants (PA) and Med Techs (MT) on caregiving, med administration and passing, campus software and computer programs, and Elite Care procedures and practices
- Train new PAs and MTs as competent and caring caregivers to Elite Care standards
- Ensure new staff competency through formalized checklists and personal evaluations.
- Ensure training checklists are completed, signed off, and returned to HR before employee works the floor or passes meds
- Oversee ongoing training for all staff to maintain compliance with required annual training (12 hours), including but not limited to dementia training, residents’ rights, food handlers, first aid, areas of improvement/concern needed, and/or overall skill set
- Work with HR, CM, and Nurse to ensure proper topics are covered; Conduct in-service training as needed
- Mentor and coach staff for success.
- Monitor talent potential of staff and plan and prepare qualified individuals for opportunities for advancement.
Manage Schedules and Staffing Levels
- Set work schedules for staff, posting at least 30 days in advance (preferably 60)
- Arrange staffing to best cover the floor based on residents’ needs; rearrange as necessary and communicate changes and updates to all parties
- Take call-outs, be aware of upcoming scheduled time off and overtime, and create a plan to minimize potential burn out of staff due to working too many hours and days
- Work the floor and fill in as PA or MT as needed
- Help cover other areas as needed such as assisting in the kitchen, activities, and/or housekeeping
- Be on-call 24/7 on scheduled work days
- On call is defined as having your EC phone on your person, charged and ready to take calls at all times when on duty and being ready to come in to fill shifts as last resort
Administration, Communication, Compliance, and Documentation
- Keep up-to-date knowledge of OARs and know how to research, interpret, and apply the rules to working situations
- Understand how OARs affect your residents, job, training needs, deficiencies, and corrective actions
- Be familiar with and maintain positive relationships with your state agency representatives (DHS, APS, and Ombudsman) and other outside agencies
- Respond timely and professionally to inquiries, requests for information, and meetings.
- Work alongside the CM and Nurse to create and write service plans and assessments
- Help organize, participate in, and when necessary, conduct service plan meetings with family members, residents, and/or POAs
- Respond to inquiries, concerns, complaints, and/or suggestions from family members, residents, POAs and hold meetings with stakeholders when necessary
- Audit medication rooms weekly to ensure order and compliance
- Communicate any medication administration issues to CM and retrain MT staff as needed
- Communicate with doctors and other outside providers, pharmacies, facilities, and referral agencies to manage medication, admission and re-admits, and appointments
- Be ready for state survey at any time, ensuring compliance with all OARs in all areas, including but not limited to: thorough documentation for training, safety, cleanliness, kitchen standards, service plans, charting, medication management/QMAR, building maintenance and code, fire alarms and safety
- Participate in the state survey process, speak with surveyors, answer questions, and share requested information
- Keep up on all paperwork, documentation, filing, purging of files, and other miscellaneous administrative tasks
- Assist with budgets and ensure spending, labor, and overtime are kept within parameters of Elite Care standards

Pay: $21.00 - $25.00 per hour
