Valley Health Partners is hiring a Registered Nurse for the Center for Women's Medicine in Allentown, PA

Salary Range $60,000- $75,000 Annual

Job Summary

Prepares patients for examinations including basic diagnostic interviewing, rooming, administering medications and vaccines, wound care for wounds or incisions, and assisting with procedures within the physician office setting. Provides care to designated patients and care coordination across the continuum. Provides education on prescription medications and clinical concerns under the direction of a provider.

Registered Nurse Education and Experience Requirements:

  • Bachelor’s Degree in Nursing, Nursing program. Public Health or relevant field.
  • 1-year experience as a Registered Nurse
  • 3-5 years’ experience as a registered nurse. registered nurse in a Federally Qualified
  • Health Center Look-Alike.
  • Licensure: Registered Nurse
  • Training: BLS - Basic Life Support

Performs all aspects of patient care, including but not limited to patient assessment, medication administration.

  • Provides or delegates care according to VHPCHC policies. • Implements individualized patient care program within the health center.

Performs patient care and procedures utilizing sound clinical judgment and approved policies/ procedures/protocols/techniques, equipment, and supplies.

  • Participates in clinical meetings and other multi-disciplinary quality service improvement initiatives.
  • Stays abreast of new trends and innovations in the areas of disease prevention and treatment, chronic diseases.
  • Compiles and submits necessary medical/statistical reports and data to internal departments or external partners in a timely fashion and, if applicable, as required by law.
  • Actively demonstrates a working knowledge of nursing theory, techniques, principles, and practice in order to holistically care for the patient.

Assists and instructs in patient and family teaching, health education and promotion and disease prevention.

  • Acts as a resource for patient and family members, listens effectively, and gives and receives constructive feedback.
  • Provides high quality care in-person and over the phone.
  • Assists patients in navigating the health care system. Coordinates specialty care, follow-up on test results and other care coordination needs.

Utilizes appropriate and acceptable intervention behaviors and communication styles in order to effectively facilitate teamwork.

  • Complies with monitoring and documentation of daily activities and expenditures as per standards set forth by accrediting bodies.
  • Participates in quality improvement and infection control initiatives and activities.

Intervenes to prevent, minimize, or correct actual or potential patient risks based upon observation of physiological, behavioral, and environmental factors.

  • Cleans and sterilizes instruments and equipment and maintains supplies.
  • Facilitates coordination of care across settings for a designated population ensuring that labs, studies, and referrals are completed and results discussed with the patient.
  • Identifies cultural/ethnic requirements to reduce anxiety and ensure patient satisfaction.
  • Processes knowledge and competence of all medical equipment utilized in the care of the patients.
  • Performs other related duties as assigned by the Leadership Team.

Job Type: Full-time

Pay: $60,000.00 - $75,000.00 per year
