Quality Assurance and Coding Nurse Job at Phoenix Home Care and Hospice
Phoenix Home Care is currently seeking a Quality Assurance and Coding Nurse for a remote position!
As the QA representative for Phoenix Home Care, this candidate must be highly organized, detailed-oriented, be able to multi-task and think on their feet in a fast-paced, demanding and rewarding position. Must have a good phone presence, be able to handle a high volume of work daily, and have excellent attendance.

Job Duties:
Responsible for review of OASIS and Home Health Plan of Care with regards to quality, Accuracy and Compliance with all guidance, regulations and agency policy. Required to work at least 40 hours per week and/or meet QA productivity standard of at least 40 chart units per week.
- Conduct review of Face to Face documentation for compliance and consistancy.
- Chart reviews to be submitted within 48 hours of assignment.
- Obtain additional information or clarification from physicians, facilities or clinical staff as needed.
- Review OASIS for improvement in quality of patient outcomes.
- Maintain above 90% in accuracy in OASIS and care planning.
- May be required to attend mandatory meetings, training and case confrenct/IDT.
- Adherance to all PHC policies and procedures.
- Efficient with time and review process to ensure maximum accurace and productivity.
- Detail Oriented.
- Knowledge of Home Health Oasis data set and/or Hospice Item Set.
What does it take?
- Minimum of 21 years of age.
- 4 years minimum of Home Health Experience
- Registered Nurse, physical therapy or Occupational therapy that is licensed in the start where practicing.
- Medical Coding experience and/or knowledge is preferred but not required
- Oasis Certified (HCS-O, COS-C) or ability to obtain within a year is preferred but not required.
- Positive attitude and self motivated.
- Minimum 2 years experience as a registered professional with at least 1 year in a home care setting.
- Clinical Assessment training.
