Job Summary:
This position is responsible for the development, implementation, and facilitation of the Service Plan process for persons served in the ICF, ensuring integration of service planning through the involvement of the individual, family, guardian, other service providers, agency staff, and others identified by the individual. The Qualified Intellectual Disability Professional (QIDP) is a staff who is responsible for integrating, coordinating, and monitoring each participant’s treatment and who possess at least three years of experience working with people with autism and/or other developmental disabilities.

Job Summary:
The QIDP plans, coordinates, integrates, and monitors the active treatment process that moves individuals with intellectual and developmental disabilities toward independence utilizing a person-centered approach to assist individuals in fulfilling their desires and needs for an independent life. This position is part of the supervisory management team that guides the overall direction of the ICF homes and is vital to fulfilling the mission and purpose.

Supervision Exercised:
Indirect supervision of all client caregivers as they implement the Individual Service Plans for all persons with developmental disabilities living at the Metzenbaum ICF.

Duties and Responsibilities:
Oversees and is ultimately responsible for completing the annual Comprehensive Functional Assessment for each client (e.g., coordinating and verifying information from all necessary disciplines to meet client needs), as well as administering the Developmental Disabilities Profile (DDP).
Chairs the interdisciplinary team and coordinates services from all internal and external support services (e.g., speech, medical, pharmacy, social services, psychology, psychiatry, vocational, educations, recreational, occupational therapy, physical therapy, dental, dietary, etc.) in order to meet client needs and ensure quality care of care. Coordinates scheduling of team meetings (times, dates, locations, etc.) for initial admission, annual, quarterly reviews, and special team meetings, informing all team members.
Completes a comprehensive annual Individual Service Plan (ISP) with input from all services; evaluations of each individual’s needs, strengths, preferences, and abilities; monthly analysis of status, needs, Unusual Incident Report trends, and recommendations.
Assures that ISPs stay updated with what is important to, and for, each individual.
Independently manages and coordinates overall development and consistent implementation of person-centered active treatment programming (e.g., ISP, individual behavior guidelines, etc.). Develops individual training goals and objectives, prioritizing goals and objectives based upon assessment and team recommendation. Collects, analyzes, and maintains data from goals and objectives no less than monthly and makes changes to, or modifications of, programs on ongoing basis.
Manages the implementation of program planning by ensuring all staff are trained in effective delivery of training techniques/methods, ensures active treatment services and documentation meets Medicaid and licensure standards and requirements (e.g., develops forms, reviews program to determine effectiveness/compliance, discusses individual progress with service providers). Documents events and discussions to anticipate future oversight and team needs.
Conducts routine rounds of residential areas to monitor and ensure all service sites deliver verifiable active treatment services and documentation in compliance with contractual, legal, and regulatory requirements (e.g., Medicaid, licensure, DODD Administrative Rules and policies, etc.) to encourage continuous quality improvement. Provides information to pre-survey & survey teams (e.g., answers questions, locates and explains documents); participates in writing and implementing plans of compliance to correct cited deficiencies; recommends proposed changes in policies and procedures.
Attends periodic training opportunities for professional growth.
Engages guardians and families as active members of individuals’ teams. Proactively communicates to build trusting relationships with guardians. Obtains consent from guardians when needed. Ensures individual and guardian participation in development of service plan and personal futures plan.
Develops positive professional relationships with DSPs, supervisors, managers, nurses, administrators, and community partners. Invests in relationships and engages DSPs as critical members of interdisciplinary teams. Communicates with team members to identify and problem-solve new issues that arrive for individuals.
Monitors all incident reports to ensure patterns of incidents are addressed. Shares all incident reports with appropriate external agencies when necessary and appropriate.
Promotes the use of positive and trauma responsive approaches to behavior support and mental wellness for individuals. Communicates individual needs and progress with staff, psychiatrists, and other consultants and team members.
Models appropriate interactions & interventions (e.g., teaching strategies, prompt hierarchies, behavior management techniques) with individuals and staff.
Benefits Management: Assistance with patient liability, Social Security, and monitoring individual account balances to ensure ongoing Medicaid eligibility.
On-call responsibilities - participates in an on-call rotation for Unusual Incident Reporting and notifications.
Provides direct care as needed.
Other duties as assigned.

Job Requirements

Minimum Qualifications
Educational/Training Requirements:
Position requires a four-year degree in a Human Services profession, Special Needs, Education, or related field as specified in the Federal Medicaid Regulations.
Experience:
At least 2 years with individuals with intellectual/developmental disabilities.
Supervisory Experience: 1 year
Licenses/Certifications:
Ensures all essential training requirements are completed, i.e., annual training, CPR/First Aid, certified medication passing, etc. Upon hire successful completion of Delegated Nursing at appropriate required level (I, II, III).
Maintain both a valid Ohio Drivers License, and a driving record that meets Agency insurance company standards (i.e. no more than 2 incidents over a 3-year period) in addition to DODD non-medical transportation rule.
Maintain personal auto liability insurance at the level that the Agency and the insurance carrier require.
Must have a reliable personal automobile available on a daily basis.

Position also requires a basic knowledge and ability to operate Microsoft office products, the internet, email, and general office equipment.
