GENERAL SUMMARY
School nursing practice requires the combination of professional clinical nursing practice with a specialized component that promotes the health, well-being, social-emotional learning, and academic success of the school-age child in the least restrictive environment. The Hawaii Keiki (HK) Psychiatric-Mental Health APRN-Rx works within a School-Based Health Clinic and is called upon to work closely in collaboration with HK personnel - Registered Nurses (RNs), Advanced Practice Registered Nurses (APRNs), School Health Support Staff (SHSS), Behavioral Health Technicians (BHTs), Administration, and others; Hawaii Department of Education (HI DOE) school personnel - School Health Aides (SHAs), Teachers, Educational Assistants (EAs), Para-professional Tutors (PPTs), Administration, Office Staff, and others; and HI DOE School-Based Behavioral Health (SBBH) personnel - Behavioral Health Specialists (BHSs), School Counselors, Clinical and School Psychologists, Social Workers (SWs), and others to carry out a wide range of school health activities designed to address student’s psychiatric-mental health/behavioral health needs. The person in this position is responsible to establish a leadership role, train, clinically advise, and provide consultation and support to HK, School, and SBBH personnel, as well as develop and provide services to students with psychiatric-mental health/behavioral health challenges and refer them to school or community mental health/behavioral health services.
Meeting of the employee’s professional responsibility will be in line with the duties described below and in coordination with the Employee’s supervisor. It is expected that the Employee will meet their professional responsibilities to the school site and program which may include attendance at scheduled meetings during non-student days.
The candidate selected for this position must reside in Hawaii or relocate to Hawaii before their start date.
ESSENTIAL JOB DUTIES AND FUNCTIONS
Assessment, Diagnosis, Outcome Identification, Planning, Implementation, Evaluation, and Revision of the following:
- Resource Mapping: Provide HK staff with updated statewide behavioral health resource map.
- Registration and Consents: Ensure students are registered for the HK Program and have a HK consent signed by parent/legal guardian or youth if age 14 or older and FERPA consent signed by parent/legal guardian and if not, then delegate this duty to the BHT, SHSS, SHA, RN, or APRN.
- HIPAA and FERPA: Comply with Health Insurance Portability and Accountability Act (HIPAA) and Family Educational Rights and Privacy Act (FERPA) rules and regulations when sharing Protected Health Information (PHI) students’ mental health/behavioral health information with authorized parties in order to ensure appropriate delivery of services and continuity of care.
- OSHA: Comply with Occupational Safety and Health Administration (OSHA) rules and regulations.
- Service Delivery: Provide services at school, home, or via telehealth.
- Evidence-Based Practice: Incorporate evidence-based practice into all activities.
- Therapeutic Environment: Promote the ability of school-based health clinics to provide mental health/behavioral health services in a safe, comfortable, confidential, and therapeutic environment, identify barriers to improve the mental health and well-being of students.
- Trauma-Informed Care: Provide services using a trauma-informed care approach.
- Crisis/Non-Crisis Interventions: Organize and develop flowcharts, policies, and procedures and interventions for management of students in both non-crisis and crisis/emergency situations.
- De-escalation: Utilize Non-Violent Crisis Intervention Crisis Prevention Institute (NCI-CPI) intervention techniques to de-escalate anxious, emotionally distraught, agitated students or parents/legal guardians.
- Mental Health Screening: Monitor HK program mental health screening processes and assist with updates as needed, conduct mental health screenings, interpret, and document results.
- Delegation: Delegate nursing care tasks, functions, and activities to RNs, APRNs, and Unlicensed Assistive Personnel (UAP) (SHAs, SHSSs, and BHTs in accordance with the regulations of the Hawaii Nurse Practice Act.
- Diagnostic Evaluations: Conduct brief psychiatric-mental health diagnostic evaluations and determine diagnosis based on DSM-5 criteria.
- Therapy/Counseling Services: Organize and provide interim therapy/counseling services to students, family, and groups assisting them toward assuming responsibility for their individual and family mental health, evaluate outcomes, and revise as needed.
- Psychotropic Medication Management: Assess students’ need for psychotropic medication, make recommendations, prescribe if indicated, refer to community prescribing providers, and monitor for the efficacy and side effects/adverse effects with regard to the student’s health status and ability to benefit from the educational program.
- Medical Report Interpretation: Review and interpret mental health reports from medical sources for school personnel.
- Treatment, Safety, and Continuity of Care Planning: Participate in treatment planning for students to address symptoms, safety issues and ensure continuity of care.
- Educational Program Recommendations: Make strength-based recommendations for modifications of student’s school program and/or accommodations for students.
- Referral to Services: Organize and develop processes to refer students to school or community mental health services, primary care provider, crisis, or emergency services as needed.
- Follow-up/Case Management: Provide follow-up and case management for students with mental health challenges.
- Documentation: Document services in the appropriate electronic health record.
- Mental Health Wellness Activities and Psychoeducational Groups: Organize and provide mental health wellness activities and conduct psychoeducational groups.
- Consultation/Recommendations/Support: Serve as a consultant and provide recommendations and support to school personnel, HK personnel, and SBBH personnel as needed.
- Content Expert: Serve as a mental health content expert as needed.
- Professional Development and Training: Provide professional development and training for school personnel, HK personnel, and SBBH personnel on relevant mental health topics, in collaboration with relevant UH, DOE and DOH partnerships.
- Liaison: Serve as a mental health liaison to programs and agencies offering field experiences for students in health sciences professions as needed.
- Precepting/Mentoring: Serve as a preceptor or mentor as needed.
- Quality Improvement, Pilot Programs, Data Collection, and Program Evaluation: Organize, develop, and implement quality improvement efforts, assist in implementation of pilot programs, data collection, and evaluation of program outcomes.
- Program Reports: Provide program reports when requested.
- Health-Related Policies, Procedures, Goals, and Objectives: Contribute to the development of health-related policies, procedures, goals, and objectives for the HK Program.
Interdisciplinary Collaboration
- HK Personnel: Collaborates with HK personnel - RNs, APRNs, SHSS, BHTs, Administration, and others.
- HI DOE School Personnel: Collaborates with HI DOE school personnel - SHAs, Teachers, EAs, PPTs, Administration, Office Staff, and others.
- HI DOE SBBH Personnel: Collaborates with HI DOE SBBH personnel - BHSs, School Counselors, Clinical and School Psychologists, SWs, and others.
- Multi-Disciplinary Team: Collaborates with multi-disciplinary teams and participates in meetings and committees as needed.
- Community Mental Health Partners and Primary Care Providers: Collaborates with community mental health partners and primary care providers to ensure continuity of care.
- Annual Back to School Meeting: Collaborates with SHAs on annual Back to School Meeting.
MINIMUM QUALIFICATION AND REQUIREMENTS
- A Master’s degree in Nursing with specialty training as a Child-Adolescent OR Family OR Across the Lifespan Psychiatric-Mental Health Nurse Practitioner (PMHNP) OR Clinical Nurse Specialist (CNS) from a school accredited by a recognized as an accrediting program.
- Current, unencumbered APRN-Rx license in the state of Hawaii.
- Current Board Certification as a Child-Adolescent OR Family OR Across the Lifespan PMHNP OR PMHCNS from the American Nurses Credentialing Center (ANCC) or other nationally recognized board certification center.
- Current CPR-BLS/ Healthcare Provider certification.
- Experience with the use of electronic health records and reporting.
- Experience as a Child-Adolescent OR Family OR Across-the-lifespan PMHNP-BC, APRN-Rx or PMHCNS-BC, APRN-Rx.
- Advanced computer skills and experience with operating telehealth platforms and documenting in electronic health record systems.
- Familiarity with and able to abide by HIPAA, Protected Health Information (PHI) Patient Confidentiality Laws, rules and regulations, and OSHA regulations for bloodborne pathogens.
